There are risks for adverse drug reactions with every medication, especially antibiotics. Cefepime specifically is associated with neurotoxicity that can sometimes be mistaken for a worsening infection (meningitis or UTI in the elderly) prompting providers to repeat infectious workup that may or may not be necessary. Daily Critical Care Pearls #17: Steroids in ARDS ✔️ Steroids are not effective as "salvage therapy" in ARDS when started late (After 7-14 days of ARDS) ✔️ Early use of steroids (within the first 7-14 days of ARDS onset) provides some benefit to morbidity and mortality risk ✔️ Steroids are best used in moderate to severe ARDS ✔️ Methylprednisolone be considered in patients with early (up to day 7 of onset; PaO2/FiO2 of <200) in a dose of 1 mg/kg/day and late (after day 6 of onset) persistent ARDS in a dose of 2 mg/ kg/day followed by slow tapering over 13 days ✔️ Glucocorticoid treatment blunts the febrile response; therefore, infection surveillance is recommended to ensure prompt identification and treatment of hospital-acquired infections. #criticalcare #pharmacy #medicine #clinicalpharmacy #pearls
