Performance of a commercial smart watch compared to polysomnography reference for overnight continuous oximetry measurement and sleep apnea evaluation Link: https://lnkd.in/d-sKxs-s Study Objectives: We evaluated the accuracy and precision of continuous overnight oxygen saturation (SpO2) measurement by a commercial wrist device (WD) incorporating high-grade sensors and investigated WD estimation of sleep-disordered breathing by quantifying overnight oxygen desaturation index compared to polysomnography (PSG) oxygen desaturation index and apnea-hypopnea index (AHI) with and without sleep questionnaire data to assess the WD’s ability to detect obstructive sleep apnea and determine its severity. Methods:Participants completed sleep questionnaires, had a WD (Samsung Galaxy Watch 4) placed on their wrist, and underwent attended, in-laboratory overnight PSG (Nihon Kohden) with a pulse oximetry probe secured either to a finger or an ear lobe. PSG data were scored by a single experienced registered PSG technologist. Statistical analysis included demographic characteristics, continuous SpO2 measurement WD vs PSG root-mean-square error with Bland–Altman plot and linear regression associations. Predictive models for PSG oxygen desaturation index and AHI severity were built using logistic regression with probability cutoffs determined via receiver operating curve characteristics. Results: The 51 participants analyzed had a median age of 49 (range, 22–78) years; 66.7% were male, with median body mass index of 28.1 (range, 20.1–47.3) kg/m2 with a race/ethnicity distribution of 49.0% Caucasian, 25.5% Hispanic, 9.8% African American, 9.8% Asian, and 5.9% Middle Eastern. WD vs PSG continuous SpO2 measurement in percentage points demonstrated a bias of 0.91 (95% confidence interval, 0.38, 1.45), standard deviation of 2.37 (95% confidence interval, 2.36, 2.38), and root-mean-square error of 2.54 (95% confidence interval, 2.34, 2.73). WD area under the curve receiver operating curve characteristics for predicting PSG were 0.882 oxygen desaturation index > 15 events/h, 0.894 AHI > 30 events/h, 0.800 AHI > 15 events/h, and 0.803 AHI > 5 events/h. WD plus select sleep questionnaire areas under the curve for predicting PSG were 0.943 AHI > 30 events/h, 0.868 AHI > 15 events/h, and 0.863 AHI > 5 events/h. Conclusions: The WD conducted reliable overnight continuous SpO2 monitoring with root-mean-square error < 3% vs PSG. Predictive models of PSG AHI based on WD measurements alone, or plus sleep questionnaires, demonstrated excellent to outstanding discrimination for obstructive sleep apnea identification and severity. Interesting development. The Samsung Watch has been granted De Novo from the FDA as an Over-the-counter device to assess risk of sleep apnea. It has specific requirements such as a statement that the device is not intended to replace traditional methods of diagnosis (e.g., polysomnography) nor intended to be used as an apnea monitor. Its good to see the large undiagnosed population getting help in assessing the risk to encourage them to get diagnosed. Sleep is a complex area that requires a skilled sleep medicine practitioner to consider a range of factors of which device outputs forms just one component. Not to mention the increasingly complex treatment landscape. The press release also states: The Sleep Apnea Feature is an over-the-counter (OTC) software-only, mobile medical application operating on a compatible Samsung Galaxy Watch and phone. This feature is intended to detect signs of moderate to severe obstructive sleep apnea in the form of significant breathing disruptions in adult users 22 years and older, over a two-night monitoring period. It is intended for on-demand use. This feature is not intended for users who have previously been diagnosed with sleep apnea. Users should not use this feature to replace traditional methods of diagnosis and treatment by a qualified clinician. For many people, the answer is “a lot.” Americans are desperate for better sleep — and they’re spending more and more money to get it. The “sleep economy,” which includes everything from mattresses to wearable devices to supplements and more, is booming and is projected to reach a record high of $585 billion for 2024. Within just a few years, Statista managed to establish itself as a leading provider of market and consumer data. Over 1,000 visionaries, experts and doers continuously reinvent Statista, thereby constantly developing successful new products and business models. For example, the global sleep apnea devices market, one subset of the sleep economy, is estimated to be worth $13.5 billion in 2024, and it’s expected to keep growing. When you look at how many people struggle with sleep, it’s not surprising they’re willing to try all kinds of solutions. More than a third of U.S. adults slept worse in 2023, according to a survey by Sleepfoundation.org. At the same time, there’s more awareness of how poor sleep impacts physical and mental health. A decade ago, sleep deprivation was deemed a public health epidemic, and many studies since have confirmed how crucial sleep is. These factors seem to be driving an openness and willingness to try new solutions. Much of the growth in the sleep market is driven by new tech products: venture capital funding for sleep tech nearly doubled between 2017 and 2021. One-third of Americans have tried a sleep tracker, according to a 2023 American Academy of Sleep Medicine survey. AASM sets standards and promotes excellence in sleep medicine health care, education, and research. These devices, which typically run between $200-$300, monitor physical signs, such as heart rate and body movement overnight. People can use this information to try to optimize their own sleep. For example, the tracker might tell them they consistently get more sleep if they avoid alcohol before bed or wear an eye mask. Impact of Home WiFi Router Exposure on the Blood-Brain Barrier and Its Effects on Sleep and Circadian Rhythms Introduction In the digital age, the omnipresence of WiFi and other wireless technologies is undeniable. As WiFi becomes increasingly integral to everyday life, concerns have arisen about the potential health effects of prolonged exposure to electromagnetic fields (EMFs) emitted by devices like home WiFi routers. The Blood-Brain Barrier and Electromagnetic Exposure The BBB is a critical structure in the brain that functions to protect the brain from toxins and pathogens while regulating the transport of essential nutrients and neurotransmitters. Recent studies have begun to explore whether EMFs from WiFi routers could affect the permeability of this barrier. Although the evidence remains inconclusive, some laboratory studies suggest that high levels of EMF exposure can increase the permeability of the BBB, potentially allowing harmful substances to enter the brain tissue. WiFi Exposure and Sleep Disruption Sleep is regulated by circadian rhythms, which are influenced by various environmental cues, including light exposure and possibly EMFs. Research into the effects of EMF exposure from WiFi routers on sleep patterns is still in its early stages, but some studies have reported that such exposure can alter brain activity during sleep, leading to disruptions in sleep quality and duration. These disruptions can be particularly concerning as they are linked to a range of health issues, including impaired cognitive function and increased stress response. Circadian Rhythms and EMFs Circadian rhythms are 24-hour cycles that are part of the body’s internal clock, running in the background to carry out essential functions and processes. One of the key concerns about WiFi exposure is its potential to disrupt these rhythms. Preliminary studies have indicated that EMFs may affect the production of melatonin, a hormone that regulates sleep-wake cycles, which could lead to circadian rhythm disorders. This disruption could not only affect sleep but also other body systems like metabolism, immune response, and mental health. Current Research and Future Directions The current body of research on the impact of WiFi router exposure on the BBB and circadian rhythms is growing but remains limited by small sample sizes, varying exposure levels, and short study durations. More robust, long-term studies are needed to conclusively determine the risks and mechanisms of WiFi-related EMF exposure on the human brain and sleep patterns. Conclusion As WiFi usage continues to grow, understanding the potential health implications of its EMFs becomes increasingly important. Shift in Sleep Patterns May Warn You About Diseases Link: https://lnkd.in/dnPgrVP8 Changes in sleep patterns can indicate chronic conditions like diabetes and even infectious illnesses such as COVID-19, a new study finds. A study published in the npj Digital Medicine examined data from 33,000 people for 5 million nights of sleep. The data was collected from Oura Ring, a smart ring that tracks sleep, heart rate, activity, and other health information. Based on the analysis, the researchers identified five main sleep phenotypes, which can be further divided into 13 subtypes. They found that people would often shift from one sleep phenotype to another, reflecting a change in an individual's chronic health conditions, such as diabetes and sleep apnea, or illnesses such as COVID-19 and the flu. Researchers say the pattern and frequency of switching between sleep phenotypes could provide two to ten times more information relevant to detecting health conditions compared to a person's average sleep phenotype alone. "We found that little changes in sleep quality helped us identify health risks. Those little changes wouldn't show up on an average night, or on a questionnaire, so it really shows how wearables help us detect risks that would otherwise be missed," said Benjamin Smarr, one of the study's senior authors and a faculty member in the Jacobs School of Engineering and Halicioglu Data Science Institute at the University of California San Diego. What are the five sleep phenotypes? The five sleep phenotypes identified in the study are the following: - Phenotype 1 involves getting about eight hours of uninterrupted sleep for at least six days in a row. Recommended by the National Institutes of Health, it is the most common type in the study. - Phenotype 2 is characterized by sleeping continuously about half the nights, but only sleeping for short periods of time in bouts of less than three hours the other half. - Phenotype 3 entails sleeping mostly continuously but experiencing interrupted sleep around one night each week. The interrupted night is characterized by one period of relatively long sleep of about five hours and one period of short sleep of less than three hours. - In Phenotype 4, people sleep mostly continuously but experience rare nights in which long bouts of sleep are separated by a mid-sleep waking. - Phenotype 5 is the rarest and represents extremely disrupted sleep, as it involves sleeping for very short periods of time every night. #sleep #research #sleephealth #health #heathcare #sleep2024 #sleeptrends #hme #sleepapnea #osa #cpap #phenotype
