This changed my life and my practice… A huge barrier in this work is in our headspace. Our difficult and so our “marketing strategy” sucks. We want to talk ACPing. The person is in “fight/ flight/ fawn… Going through “WMTY” is kind, curious and connects for a journey. Easily nuanced to be trauma informed. So all can define their own “difficult”. “ACP” happens organically when we ask about living and listen deeply. For however short or long the time we have to live. We can plan to live that space. Advance care planning is the process of discussing and documenting our preferences for medical care in case we are unable to make decisions for ourselves. It ensures our values, wishes, and preferences are respected and helps guide our loved ones and healthcare professionals in making decisions on our behalf. Yet, these discussions still do not occur as often as they should. I find myself having these conversations with patients too far upstream. SIDeR+ provides an integrated digital record from Somerset NHS, GP practices, Somerset Council, and more. It ensures care teams have instant access to up-to-date info, cutting down on admin time and enabling better, faster care for patients.
