Kris Kristofferson, Diagnostic Quality Warrior? Yes. The family of Kris Kristofferson announced he passed away September 29, 2024. The well-deserved accolades for his career as a songwriter, singer, actor and activist will fill our news outlets and social media pages. I was, and will remain, a fan. But what I will always admire Kristofferson for is sharing his important story of how he was multiply misdiagnosed and harmed by it. Over a thirty-year period, Kristofferson was treated for fibromyalgia, blinding headaches, “golf ball-sized, painful” muscle spasms, sleep apnea, knee pains, arrhythmias, and anemia due to too much Tylenol. Then he experienced memory loss. His medical team were treating each of his symptoms individually, not recognizing they were all indicative of something else, a single disease that caused all his symptoms. Lyme Disease. Medical and Mental Health professionals are mistaken about me having cognitive dissonance regarding my smoking cigarettes; especially since, I have pulmonary fibrosis. What I know you understand; but, refuse to acknowledge is, I also have hereditary chronic pancreatitis because of a SPINK1 mutation and get to look forward to an excruciatingly painful end by pancreatic cancer. I choose to take my chances and try to get lung cancer and die from that before the pancreatic cancer kicks in. And, I INSIST you allow me to choose my death with DIGNITY! Let me choose the lesser of two evils; because, you know hereditary chronic pancreatitis is a degenerative disease with no known cure. So, STOP telling me you refuse to treat me for anything unless I quit smoking. STOP telling me palliative care is too expensive for me to receive. STOP denying me a comfortable end; because, you choose to apply your medical and personal biases to this unique set of circumstances. And, STOP treating me like I'm stupid. I'm highly intelligent, knowledgeable of the latest research into treatments, and mentally competent to make my own informed decisions.

The 17th #RareDiseaseDay took place on February 29, 2024 under the motto “#ShareYourColours”. By definition, a rare disease affects no more than 5 in 10,000 people. It often takes a very long time for patients to receive the correct diagnosis; for some, this never happens. Treatment options are also often limited because too few participants can be recruited for clinical studies and research funding is missing or not provided. For personal reasons, I would like to take this opportunity to point out the rare disease #HSP - Hereditary Spastic Paraparesis (between 1 in 11,000 to 77,000 in Europe). The disease can take many forms and be the result of different genetic abnormalities. All forms result in degeneration of the nerve pathways that carry signals from the brain to the spinal cord (muscles). Depending on the form, the first symptoms of hereditary spastic paraplegia can appear at any time, from the first year of life to old age. Reflex movements become excessively strong, leg cramps, twitches and spasms make the affected person's gait stiff and jerky (spastic gait), so that walking ultimately becomes very difficult and in some cases even impossible. Unfortunately, the treatment options are still limited to alleviating the symptoms and the progression of the disease and consist of physical and occupational therapy, individual aids (e.g. orthoses and wheelchairs) and drug therapies (e.g. botulinum toxin). Two of my children and myself live with HSP. It took 10 years to identify the HSP in our case. Hopefully better treatment options will be discovered in the next 10 years. What happens if your vital lung capacity shrinks over time? The vital lung capacity (VLC) is the total amount of air expelled in a maximum exhalation. On average, the VLC ranges between 3.5 to 4.5 liters and is affected by factors such as age, height and sex. Why should we care about our vital lung capacity? During a large medical study, The Framingham Heart Study, scientists monitored 3 generations of people to identify the factors that lead to cardiovascular diseases. One of the conclusions after the first 20 years in the study was that lung volume, aside from age, was the most accurate predictor of whether someone would become ill. Our vital lung capacity shrinks as we grow older and for the longest time, doctors thought that you could not “train” your vital lung capacity. However, inspiratory muscle training (IMT) is a real thing, and there are many cases of people increasing their vital lung capacity through the daily practice of breathing against resistance, as well as body and stretch exercises that increase the flexibility of the breathing muscles. There is a lot of talk recently about longevity and living forever. Since only a few can afford millions a year on doctors like Bryan Johnson, focusing on improving your vital lung capacity is possibly the most effective way to improve your life quality, and it does not have to cost a penny!

Grace Bennett, an employee at Breakthrough T1D (formerly JDRF), had always been fiercely independent, navigating the ups and downs of life with Type 1 Diabetes (T1D) that ultimately caused her to have Diabetic Retinal Disease (DRD). When she began experiencing complications with her vision as a result of DRD, everything changed. The fear was overwhelming—what would happen if she lost something as crucial as her eyesight? On top of the fear, there was the crushing weight of shame and isolation. Grace couldn't shake the feeling that it was her fault, that maybe she'd done something wrong in managing her diabetes. Her retinal specialist assured her it was not her fault, telling her that living with T1D for over 20 years—like in her case—very often leads to serious complications like kidney and heart disease as well as vision loss from DRD, even when you do all you can to manage blood sugars. At that moment, Grace felt a wave of relief. This understanding gave her the strength to focus on addressing the complication and moving forward. There's still so much more work to be done. We must continue to support innovative research and develop new therapies to address the early stages of DRD and restore vision in people who have lost it due to diabetes.
