In the latest issue of the Journal of Clinical Pathways, Andrew Pecora, MD, and colleagues, provide insights and perspective from each constituent involved in shaping health care payment and delivery throughout the US. "Self-imposed problems have tied the health care industry into knots with no clear path to untie them. We cannot change laws restricting innovation if we ask patients and physicians to reduce total cost of care without proper support and appropriate incentives for clinicians and other care providers to ensure achievement of the highest quality clinical outcomes," said the authors. Pretty good study about the patient-doc experience in the USA. The issue of cost is clearly still a major theme. #healthcare * Most (73%) claim to have had a physical in either 2023 or 2024. * Of those who haven’t, scheduling challenges, and aversion to doctors are the biggest reasons given. * 1-in-4 have skipped medical care (of any kind) because of cost. * Patients want their doctor to have people skills: be a good listener, be understanding, be able to ease their concerns. * Patients generally follow their physician’s advice, but when they don’t, its often because it is difficult to implement in daily life, or because of cost.

Services like dental care, postpartum care and gender-affirming care are supposed to be essential health benefits (EHB’s) covered by all plans. Yet it has become increasingly clear that these benefits aren’t as comprehensive as intended to be, leaving many people without access to critical services. "And what we need is federal action to update essential health benefits," said Adam Fox with Colorado Consumer Health Initiative, "ensure that the full range of whole person care is covered, and that that is made a requirement across the country." "We're going to digitize...going to ePA..." but before you do that let's make sure your PA process is well streamlined and furthermore, CMS has cut the timelines by about half on PA decisions (~72h for Urgent PA decisions & 7 Calendar days for Non-Urgent decisions). This needs to be done by the beginning of 2026 and the ePA by beginning of 2027. PA is just one piece of the puzzle. Metrics will need to be provided/reported. Where are you in your process to get all this in place? Too much at stake to play a game of "Healthcare Jenga". A few weeks ago, I posted an article about older adults often don't tell their #PCPs about seeing a #DTC telemedicine provider. This has repercussions for continuity of care. While I still advocate for being honest with your doctor (better to know other meds you might be taking), let's also remember why this is happening in the first place. If someone can't reach their doctor or these services provide easy ways to get care, people will use it. These were the top use cases: - #Mental health: 12%. - #Sexual health: 15%. - #Weight management: 9%. - Skin care: 9%. - Hair loss: Approximately 5%. - #Pain management: Approximately 5%. The cause is how our health system (dys)functions so that people have to seek care outside of their dedicated team. There are better solutions, certainly, but DTC fills a gap that will require a deeper healthcare fix.
