The introduction of the March episode of APIC's 5 Second Rule Podcast is a full circle moment for me whereby I had the pleasure of interviewing the women in that video, world-renowned patient advocate Rosemary(Rosie) Bartel in observance of Patient Safety Awareness Week. Here's a breakdown of its indications, procedure, and risk mitigation: **Indications:** ISC is commonly recommended for individuals with urinary retention, neurogenic bladder dysfunction (such as spinal cord injury or multiple sclerosis), urinary incontinence, or post-surgical bladder management. **Procedure:** During ISC, patients insert a clean, lubricated catheter into their urethra to drain urine from the bladder. It's typically performed several times a day as needed to ensure adequate bladder emptying. **Risk Mitigation:** Proper technique and hygiene are essential to minimize the risk of complications. Patients should receive thorough training from healthcare professionals on catheter insertion, maintenance, and infection prevention. Using sterile catheters, maintaining cleanliness, and staying hydrated can help reduce the risk of urinary tract infections (UTIs) and other complications. New Insights in Complicated Intra-abdominal Infections: IDSA Guideline Update 2024: The Infectious Diseases Society of America (IDSA) has just released an updated clinical practice guideline for diagnosing and managing complicated intra-abdominal infections in adults, children, and pregnant people. Key Takeaways: Risk Assessment: The new guidance emphasizes the importance of risk stratification using scoring systems like APACHE II for adults to predict mortality in patients with complicated intra-abdominal infections. Imaging Updates: For suspected appendicitis, ultrasound is recommended as the initial imaging modality in children, while CT remains the preferred first choice for adults. MRI is recommended as a secondary imaging option for equivocal cases in both children and adults. In pregnant individuals, ultrasound is the preferred initial imaging modality for suspected appendicitis, with MRI as a reasonable alternative if readily available. Blood Cultures: Blood cultures are recommended in adults and children with suspected intra-abdominal infections who have fever AND hypotension, tachypnea, or delirium, or when concern for antibiotic-resistant organisms exists. In non-immunocompromised patients without these symptoms, routine blood cultures are not recommended. Fluid Cultures: Intra-abdominal fluid cultures are recommended in patients undergoing source control procedures for complicated intra-abdominal infections, but not routinely during appendectomy for uncomplicated appendicitis. This updated guidance provides clinicians with the latest evidence-based recommendations to optimize patient care and improve outcomes.
