As an instrument technician part of my responsibility is to ensure that I return equipment back to safe working conditions when they fail.So today i carried out a corrective maintenance on this flow switch. this flow switch is attached to the blower of an Electro-chlorinator. who can tell me more about an Electro-chlorinator. Today, I worked alongside a field instrument technician to install a radial vibration probe on a running compressor. Even though the sensor was validated and the gap voltage was perfect, it still wasn’t picking up the vibration. Together, we worked through the issue and found a solution. Later in the day, the technician sent me a thoughtful message thanking me for my support. It was a good reminder that knowledge sharing works both ways. Sometimes, it’s about working together, combining insights, and learning from each other. What may seem simple or routine to you could be exactly what someone else needs. We’ve all had moments where a little guidance or shared knowledge made all the difference. So, don’t hesitate to share what you know—even if it seems small. I’ve recently decided to expand my studies on root cause analysis (RCA) of instrument devices. The point where we learn how to analyze the reasons of an unwanted incident and identify the proper solutions in order to solve the problems so the operatig plant remains more safe, reliable and maintenance friendly! I’ve tried to mix my own experience with scenarios based on true stories. Today, I’m gonna investigate the causes of a plant trip due to performing maintenance on an incorrect tag number. A simple pressure transmitter triggering a pump interlock logic on its low low value needed maintenace due to fluctuations seen in its pressure value. A total of three pumps were installed in the same area for the same purpose by the way and all of the corresponding transmitters of these three pumps were installed on a single skid together and during the maintenance on the pressure transmitter of pump A, pump B suddenly tripped. What can be the reasons for this happening? The instrument technician had actually verified the pressure transmitter on which the job was to be done since the tag number of the pressure transmitter (PT-A) matched the tag number mentioned on the work permit (PT-A) but as soon as the technician isolated the manifold of the pressure transmitter, the pressure of pump B dropped to 0.5 bar on the operator HMI. At the same time, pump B tripped. Hence, work on the incorrect tag number was absoulutely clear. Further checking revealed that the impulse tube of the pressure transmitter is connected incorrectly to pump B pressure transmitter and vice versa! how to avoid such problems? 1. Usually, the cable of every transmitter has an stainless steel tag plate. In this case, the tag plate was missing. 2. The display of some transmitters can be configured to show the tag number also. In this case, the tag number on the local display was left blank. 3. Cross checking the value of the process value shown on the local display with the control room operator who’s monitoring the same value on the DCS HMI will eliminate such incidents. 4. In this case, a common skid was designed for mounting all transmitters. Curious question...any and all form of Instrumentation Technician, DCS / Controls (3 or more years of field experience). When conducting a loop check, have you ever NOT had documentation on the device being tested? No DCS support, nor RATS list or at minimum I/O list at the board to mark off progress? Did you not record the results during each step of the test? Have you tested an SIS device using a foreign source for supply pressure? Without an approved deviation? Didn't conduct the SIS timing test to make sure the fail / response timing was within specified duration?
