key aspects of geriatric care: 1- Comprehensive Assessment and Personalized Care Planning: Geriatric care takes a holistic approach to health assessments for older adults, covering physical, mental, functional, and social well-being. By developing personalized care plans tailored to individual needs, this approach ensures proactive, comprehensive care focused on maintaining independence and quality of life. 2- Prevention and Management of Complex Health Conditions: Geriatric care targets preventing and managing common health conditions in older adults like falls, dementia, depression, polypharmacy, and frailty. Geriatricians optimize medication regimens, manage pain, and promote healthy habits, coordinating care and interventions to address the challenges of multiple chronic conditions. 3- Promoting Quality of Life and Functional Independence: Prioritizing quality of life, geriatric care supports older adults' physical, emotional, and social needs, aiding daily activities and social engagement. Healthcare Strategy and Innovation | Transformation for Improved Patient Care | Shaping the Future of Healthcare Delivery
Call to Action: Geriatric Care Special Needs Assessment Elderly individuals face unique health challenges that require specialized assessments to ensure they receive appropriate care and support. A thorough need assessment is essential to identify the diverse physical, cognitive, social, and emotional issues. This should also include fall prevention, nutrition, polypharmacy, and mood disorders, all of which contribute to a holistic understanding of an elderly person’s healthcare. Evaluating aged people's ability to perform basic activities and complex activities of daily living, and risk to Vaccine-preventable infections and a variety of common physical health conditions are essential for health system strengthening for accessible quality geriatric care. Early intervention for sensory impairments significantly reduces quality of life, increases social isolation, and limits their ability to interact with their environment. Additionally, regular nutritional status, food intake, and addressing issues such as difficulty chewing, swallowing, or poor appetite are crucial in maintaining the physical health of elderly individuals. Cognitive assessments for dementia, mild cognitive impairment, and memory loss, are also critical for mental health and wellbeing. Comprehensive geriatric care needs multidisciplinary assessment to ensure social engagement, emotional well-being, and preventive-rehabilitative care to improve quality of life and allow them to age with dignity and independence. #geriatriccare #QoC #UHC #accessiblecare #healthsystemsterngthening #publichealthmatters #calltoaction #India2047 #SDG #AgingWithGrace

TEDx Speaker | CEO of GeriatRx, Inc. | Certified Community Pharmacogenomics (PGx) Consultant, Deprescribing Coach and Implementation Strategist, Top 50 Most Influential Pharmacists 2022, The Deprescribing Pharmacist
Now because I am a geriatric pharmacist and I cater mostly to the elderly patient population, I have some thoughts… Okay yes - I can admit that there are some patients who aren't complaining about the amount of medications they are taking. But of this same patient group, there are complaints of at least one or more of the following: 💊High medication cost 💊Minimal access to prescribers 💊Availability of medications 💊 Medication-related side effects Look, I am not in the business of taking patients off medications just because I think they are on too many. (Although this is the main concern of most of my clients and their families.) I am in the business of reducing the risk of harmful effects of medications, reducing the risk of death, and improving the quality of life for elderly patients everywhere. Without deprescribing advocates, patients will never see the potential for cost savings, they will never understand the risks associated with long-term use of some drugs, and they will never be able to see how taking fewer medications can improve their lives. Deprescribing is the best-kept secret for keeping our older adults close and independent. Sign up for our December cohort here 👉🏾We have to make Deprescribing the Standard of Care for all older adults suffering from polypharmacy and who better than geriatric care providers to advocate! Aastha Hospice Dr. Abhishek Shukla - "Geriatric Giants", were introduced to the students which are the primary health challenges faced by elderly individuals. These are often referred to as the "Enemies of Seniors", and understanding them is essential for providing effective care. The Four Geriatric Giants include: Immobility: This refers to the inability or difficulty in moving, which is a major problem among the elderly. It can result from various causes such as muscle weakness, joint issues, or neurological problems, leading to a decline in overall physical health and increased dependency. Instability: As people age, their balance and coordination tend to deteriorate, leading to falls and injuries. Instability, can severely limit an elderly person's independence and is a significant risk factor for hospitalization. Incontinence: The loss of control over bladder or bowel function can severely affect the quality of life for seniors. Incontinence often leads to embarrassment, social isolation, and can contribute to skin infections if not managed properly. Intellectual Impairment: Cognitive decline or issues such as dementia and Alzheimer's disease fall under this category. Intellectual impairment can impact memory, reasoning, and decision-making, significantly affecting an elderly person’s daily life and autonomy. By addressing these geriatric giants, healthcare professionals can greatly improve the quality of life for older adults, ensuring they receive **holistic care** that focuses on maintaining mobility, safety, dignity, and mental well-being. sajournalofgeriatric palliative medicine #AasthaCares #lminp #GeriatricCare #Immobility #Instability #Incontinence #IntellectualImpairment #ElderlyCare #HolisticCare #sajournalofgeriatric #drabhishekshukla #aasthahospice #aasthacares

Celebrating the Depth of Geriatric Medicine- As a Geriatrician committed to delivering personalized care, I have always believed in the power of home visits to truly understand and address the complexities of elderly health. This picture of a meticulously detailed note from a dementia patient's son exemplifies why I choose this approach. Each point captures critical aspects of the patient’s health, highlighting concerns that are often overlooked in the rush of a typical outpatient setting. In geriatric medicine, active listening is more than a skill—it's an essential part of the care process. Written notes like these from family members are invaluable. They not only outline the medical needs but also reflect the dedicated involvement of family in the patient’s well-being. From medication adjustments to daily nutritional needs, these details guide our interventions and help us tailor care plans that respect the individuality of each patient. Home visits allow us the time and context needed to engage deeply with our patients and their families, fostering a therapeutic partnership that is both effective and compassionate. This I believe is geriatric care at its best, where every detail counts, every voice is heard, and every piece of information adds value to our care delivery. #GeriatricCare #HomeVisits #ProudGeriatrician #PatientCenteredCare #HealthcareInnovation #ActiveListening #GeriCare #AssistedLiving
