Identifying the root cause is the key to improve Aviation Safety. Bruce Webb gives an insight to history of the aircraft accident in 1908, what happened and how root cause analysis more than 100 years ago was picked up. Even that aviation has overcome within the past 100 years to mostly eliminate technical errors, we are confronted in the 21th century to lower the accident cause rate due to human factors to further improve aviation safety. Root cause analysis is the core in our new challenge as in history. Let’s not forget the past to shape the future, thanks Bruce for sharing your thoughts and give that virtual journey.
