Pharmacists you are needed! 👨🏻⚕️ Had a hospice patient few weeks ago being treated with glipizide, metformin and sliding scale lispro for T2DM 💊💊💊 Naturally, I was curious as the standards for treating this are very lenient in hospice Goal fasting blood glucose < 180-200 ⬇️ Past week of recent blood sugars were sub 100s, even a few low episodes ~ 60 Most recent A1C few months ago was 4.8% 😳 What I did: - Suggested to stop all agents ❌ - Suggested to stop needle sticks ❌ Overall this reduced unnecessary medications, lowered risk for hypoglycemia, and probably improved patients QOL with less sticks! #pharmacist #palliativecare #painmanagement #addictionmedicine #hospitalpharmacy #polypharmacy

Insulin pens can be significantly easier to use than syringes and vials — but there are a few limits with pens, too. Like the dosing. Sure, it's fun and easy to twist and turn that little knob, dialing up your dose with those little clicks, but what if you want to take less than 1 unit or less than .5 units? Yes, people with diabetes need teeny tiny doses of insulin sometimes! Pens don't really offer that option. That's ONE reason why I hack my insulin pen with syringes.
