Intubation is challenging! So, why make it even harder? Time and time again, video laryngoscopy has proven to be the superior method for achieving successful intubation on the first attempt. However, in certain difficult situations, if bodily fluids obscure the camera lens in the mouth, a lack of view leads to decreased accuracy. This becomes a critical period for the patient—who is not breathing—and the medical provider, who may experience increased stress. A newly published, landmark trial in the Journal of American Medical Association (JAMA) showed that GlideScope® hyperangulated video laryngoscopy outperformed direct laryngoscopy, with higher first-pass success and fewer repeated attempts. Including over 8,429 elective and emergent surgical intubations, and a range of providers and experience levels, the study concludes video laryngoscopy as a preferable initial approach for surgical intubation. Although, in my opinion, these are drastically different devices - I completely understand the need for clarity between the two. In the labs, there is a common debate between which to use.
