Maribeth Gallagher, DNP, PMHNP-BC, FAAN, shares a “eureka moment“ in the newest CAPC article, “How to Provide Excellent Dementia Care for Patients with Dementia—and Their Families.” Suppose a certified nursing assistant (CNA) needs to change a soiled brief, moving briskly for efficiency’s sake. The dementia patient struggles to interpret what is happening. They can’t process who that CNA is, what they’re doing, or why. What they do know is that nobody touches them “down there” without their permission, and they ache every time they’re moved. The patient naturally reacts with fear or anger, escalating to verbal and physical aggression. They are labeled “combative” when in reality, they are more accurately exhibiting what we are all hard-wired to be: self-protective. By simply walking through various scenarios, we can switch the lens from a “combative” person to a human being who is suffering and scared to death. That’s a real “aha” moment for some. Important study on improved mortality prediction in nursing home residents from our #icare4old study. This prediction can be integrated in assessment software and support timely identification of the approaching of the end of life: https://lnkd.in/eYr8TNSV
In a new I-CARE4OLD EU Project study, researchers from Hebrew SeniorLife examined #risk of #death in a cohort of nursing home residents in the US and Canada, resulting in the development of the DeathRisk-NursingHome scale, an effective #prediction model to identify residents at risk of death within the first 12 months after admission to the nursing home. The new tool is a refinement of the well-established interRAI Global CHESS-scale (Changes in Health, Endstage Disease, Signs and Symptoms) and may be helpful in patient care planning, resource allocation, and excess death monitoring. #riskprediction #olderadults #long-term care
