🏥💉Has anyone else had a bypass time of over eight hours? 🤔 That's how long my colleague was on bypass in a recent case, and it was a marathon! ⏰💪 Being on bypass for that long can be intense and challenging, but it's all worth it in the end to help save a life. I'm curious to know if anyone of you have had a similar experience. If you have, please share your story with me! What type of case were you working on that required such a lengthy bypass time?💡I'd love to hear about your experience and learn from your insights. I have had them and the bigger question,as you see in the stories,is why we did them alone. How can anyone maintain a blood sugar, go to the bathroom, on a case that long. How can anyone think about the best parameters that you should be maintaining for the best care for your patient. After about 3 hours, a Perfusionist needs a break, to step away, eat and go the bathroom. Then they can come back to be “intense and challenging”! It is worth it in the end but it should be a standard 24 hours a day, not just during business hours.

8 hrs: Straight forward case till weaning! Protamine Reaction, cardiogenic shock, flapped LIMA on LAD with minimal diastolic filling showed by the coronary flowmeter, return back to pump, no ECMO available, redo LAD, successful weaning, no postoperative complications, discharged at 7th day Oxygenator FX25, hypooclusive system, maintaining Hb > 8 with DO2i > 280. Cell saver is standard for all cases.

21:30 and I was solo for the whole time. About 18 hours in I did have to have a cup of coffee brought in. And the bio breaks were kneeling behind the pump into a larger mouth saline jar.

I had one case for 8hours 48 min. Paediatric case,done alone. Another one 36 hours and 42 min.shared with one perfusionist.

Liv Perfusion- Fourth time Re-do. I need to recheck. 10-12 hrs on CPB. Spontaneous conduction when X-clamp removed. No inotropes or mechanical assist going to ICU.

10.5 hrs ✋AVRx4, protamine reaction, LM dissection. Bumpy post-op in ICU. Pt went home in 12 days with no deficits ❤️🙏

The maximum I had was 9hrs..the patient had tumour in her heart..she survived but Bleed till the next day
