Collaborative working means becoming greater than the sum of your parts – with real benefits for both patients and providers. But what does that really look like in reality? In a new blog, Dr Sian Stanley discusses how her GP practice worked with community and acute partners and others to set up an urgent treatment centre in Harlow. The project was in response to the unsustainable pressures faced by A&E, and to date, the UTC has started to reduce some pressure on the local emergency department. In particular, fewer diagnostic procedures are now taking plan in A&E, equating to cost savings for the ICB. More joined up working is also making a difference for patients, with better triaging and more use of alternative care options, which are re-directing people away from A&E and enabling them to get care closer to home. This means saving time and avoiding risks associated with being admitted to hospital, particularly for older people. Great first session today at The Pharmacy Show, focused on - Implementing the recovery plan: good practice examples of integrating Pharmacy First ✅ At its 6-month anniversary since inception (Jan ‘24), fascinating to hear from colleagues at: NHS Derby and Derbyshire Integrated Care Board (ICB), Staffordshire and Stoke-on-Trent Integrated Care System, NHS Northamptonshire and NHS Birmingham and Solihull on the progress made in embedding new care pathways. A few takeaways: 🗓️ In its formerly known self ‘Community Pharmacy Consultation Service’, Pharmacy First has huge potential with anticipated appointments savings in excess of 10m every year! 👩⚕️ Incorporating the 7 most common clinical pathways, prescribing pharmacists are empowered to deal with ‘minor ailments’, therefore shifting more traditional demand away from general practice. 📺 Patient education is key - we have to support citizens to understand alternative care channels, with TV ads, bus shelter adverts and social media channels already showing a 44% increase in uptake. 🚪 Staff education is also key - training practice staff, care navigators and social prescribers, empowering them to redirect patients at (or preferably before!) the front door of general practice. 📱 There are applications out there already such as ‘Joy’ that allow self-referral into these community pharmacy channels, in addition to other channels such as 111. Empowering Vision: RNIB unveils NEW Low Vision Framework to enhance lives and foster inclusivity. At RNIB (Royal National Institute of Blind People), our commitment to enhancing eyecare services has reached a new milestone. We're delighted to announce the launch of the Adult Low Vision Service Quality Framework, a robust guide developed in collaboration with cross-sector partners. The ten-part framework covers various aspects, from the environment to staff training, providing a comprehensive benchmark for auditing and developing services. It's the result of years of collaborative effort with individuals with lived experiences, healthcare professionals, and relevant organisations. An outstanding low vision assessment goes beyond identifying aids; it involves understanding patients' needs and recommending additional services. This framework aims to elevate the standards of care for individuals at all stages of their eyecare journey. If you're involved in low vision services – be it in a clinic, community setting, or commissioning – we invite you to explore the framework and good practice guidelines on the RNIB website . Additionally, sign up for our e-Learning module on a holistic approach to low vision. The Low Vision Framework is a pivotal element of RNIB’s broader innovation in the Eye Care Support Pathway launched at the end of 2023. In another sign that North Carolina's CON law is not containing costs but rather reinforcing incumbent's market power. A "boomlet" of freestanding EDs are opening in higher income urban and suburban areas already well-served by big hospital systems Mission/HCA, Atrium, and Novant. The NC CON law restricts freestanding EDs (FSEDs) to those operated by acute care hospitals, who have already invested in dozens of health system-branded urgent care clinics in these same metropolitan areas of NC. Urgent care and freestanding EDs may help relieve hospital EDs of some low-acuity visits and demands on EMS, but evidence indicates they don't lower overall spending. It's hard to imagine new FSEDs will be easy to staff. A freestanding ED is another hospital department that levies facility fees and has higher lab, imaging, and professional costs compared to urgent care. Even worse, UCs and FSEDs undermine the viability of community-based primary care practices dedicated to preventing and managing chronic disease and establishing relationships built on trust and accountability. The British Medical Association (BMA) has released the "Safety, Stability, Hope: A Vision to Rebuild General Practice in England" blueprint, addressing the critical state of general practice and proposing actionable solutions to ensure its sustainability. 🔍 Key Statistics: ➡ General practice delivers around 1.5 million appointments daily, with 65% face-to-face. ➡ Funding for essential services is at an all-time low, equating to just 30p per patient per day. ➡ The average number of patients per GP has risen to 2,300, a significant increase over the past five years. ➡ Over 1,000 NHS GP practices have closed since 2015, despite an increase of 6.4 million registered patients. 🏥 Blueprint Goals: Safety: Immediate creation of NHS roles for GPs and Practice Nurses, including a funded fellowship post and relocation incentives. Stability: Establishment of a 2025 Family Doctor Charter and a commitment to reversing the decline in GP practice contract funding. Hope: Reworking the GP practice contract to ensure transparency and adequate investment, focusing on a preventative, community-based healthcare model. Through this plan, the BMA aims to protect and resource GPs, ensuring they can provide continuous, high-quality care and address the growing demand. 🗣️ Your Thoughts? With this month marking the Universal Care Plan’s (UCP) second anniversary, Tomas Ince, UCP Senior Clinical Transformation Manager, has shared valuable insights on the milestones over the past two years and the benefits of the integrated approach that the Universal Care Plan brings to the people of London. With key insights into what to look forward to for the future. 🚀 Having supported Better for the past three years with the integration of the Universal Care Plan for OneLondon, ReStart are extremely proud to be part of a project which is having such an impact and helping to improve patient care. As commented by Tom Ince; “As of June 2024 over 64,000 Londoners had an active UCP, with over 16,000 being created since the beginning of the year. That is the most since the UCP’s conception and the growth over the past six months has been a joy to behold.” In terms of future developments Tom goes on to explain: "We have had some great achievements in recent months, but our digital journey is never complete.
