Our residents and fellows often have questions about their contracts that they would be signing for their first employment as a fully trained IM/FM or Geriatric Medicine physician. My advice to them is based on the Patient Priorities Care model, modified to everyday life. Using an example here: 1. Identify your values: Professional, academic growth and connecting with family. 2. Bothersome tasks: long hours with unstructured schedules 3. The ‘one thing’ that is highest priority and most important to you: be able to travel for longer trips. This helps us define our goals: “become a leader in the field in 3 years and showcase my own clinical work nationally; be able to enjoy personal freedom through travel with family”. With this goal you can identify if the contract you are signing will bring value to you as you bring value to your employer! We often get entangled in identifying our 5 year goals and plans, the important thing to ask first is what are our own values and priorities. It is not so different from how we handle patient care! Mature Pathology networks such as my own must help others bring in the resources they need to roll out new technology at pace to provide the best patient care. All of it has me run off my feet.. Agree, critical testimony to hear. I think that we all can agree that the concept of an electronic medical record is great, and I love not having to wait for physical copies of MRI and CT scans, but the administrative burden that goes along with an EMR is significant. Documentation/coding requirements often also mean that a 10-page note when printed only has about 5 lines of worthwhile information and 9.5 pages of redundant information just copied and pasted from elsewhere into every single patient interaction.

Recently, I read a post by another physician online about an experience he had decades ago, soon after he first became an attending in the ER. The experience still troubled him significantly.  The anecdote was one of so many in a long career on the front lines of healthcare, where life and death are often part of the daily events, amidst the otherwise mundane and ordinary.
