However, a key to becoming a super organized circulator is to have an intentional plan. Antoine de Saint-Exupéry once said, “A goal without a plan is just a wish.” It is essential to have a strategy for being organized. As the acuity of patients continues to rise not only in inpatient settings but also in ambulatory clinics, surgery centers, and infusion centers, ISMP and a nursing task force are raising awareness about potential errors with the frequent use of IV push medications. 🔍 ASK Advocate Maggie: Clarifying Sedation Practices 🔍 🤔 Your Question: Can Anyone Provide Sedation? It's crucial to understand that not just any nurse can provide sedation to a patient. Only nurses with the appropriate education, training, and additional credentials are qualified to administer sedation safely. 💡 Insight: Sedation is a specialized nursing practice that requires specific knowledge and skills. ICU nurses, for instance, may manage patients with airways but may not necessarily be trained in conscious sedation practices, a patient with a secure airway is a far cry from a patient having a procedure and administering sedation. It's essential to know the rules and regulations in your state, often outlined in position statements, to ensure compliance. 📋 Position Statement Guidance: Refer to your state's nursing board position statements for clarity on sedation practices. These statements outline the qualifications, education, and training required for nurses to administer sedation safely and effectively. Always review organizational policies and well as national associations like AMERICAN ASSOCIATION OF Anesthesiologists to keep up to date with best practices. 🚫 Misconception Alert: Being an ICU nurse doesn't automatically qualify someone to administer sedation. It's a distinct nursing specialty that requires specific training and expertise beyond general critical care management. If your employee file and competencies are pulled do they reflect the EDUCATION, TRAINING, AND KNOWLEDGE per the NPA, policy, and best practice guidelines? As a prior investigator for a board and expert witness these are all reviewed in any litigation if there is a bad outcome. 💪 Advocate Maggie's Advice: Empower yourself with the necessary education and credentials if you're interested in providing sedation to patients. Ensure you meet the requirements outlined in your state's position statements and seek additional training as needed. After my newly-minted RN status at the Cleveland Clinic on the VIP ward was beginning to lose its luster, I transitioned into routine everyday nursing care. When dealing with a variety of patients, I had to learn the art of “keeping it real”. What’s that you ask? Here’s an example . . .  I’d just arrived for my 3 to 11 PM shift, ready to receive updated patient reports from the earlier shift. Leaving the nurses station, I felt completely professional and overly confident. Reviewing my shift report notes, I checked on my first patient who underwent surgery two days earlier. Calmly, I asked: “Have you experienced any flatulence today?” Clearly, in response to my mastery of medical terminology, his reply was teeming with frustration. “Why do all you nurses use such big words? No one can understand what you’re talking about.” Without losing a beat, since I clearly overwhelmed him with my nursing superiority, I replied in a self-deprecating manner: “ I apologize for the confusion. So, did you fart today?” His wife and another nurse were laughing so hard they had to leave the room. The patient nodded, unable to speak, as his face turned a bright shade of scarlet. I had my answer through my newly-acquired art of keeping it real. 𝐅𝐚𝐢𝐥𝐮𝐫𝐞 𝐭𝐨 𝐦𝐚𝐢𝐧𝐭𝐚𝐢𝐧 𝐩𝐫𝐨𝐩𝐞𝐫 𝐬𝐭𝐚𝐟𝐟𝐢𝐧𝐠 𝐫𝐞𝐪𝐮𝐢𝐫𝐞𝐦𝐞𝐧𝐭𝐬 𝐢𝐧 𝐭𝐡𝐞 𝐏𝐀𝐂𝐔 𝐬𝐞𝐭𝐭𝐢𝐧𝐠 𝐚𝐧𝐝 𝐧𝐞𝐠𝐚𝐭𝐢𝐯𝐞 𝐩𝐚𝐭𝐢𝐞𝐧𝐭 𝐨𝐮𝐭𝐜𝐨𝐦𝐞𝐬 ▶ Staffing standards for nursing in the PACU are crucial to patient outcomes. I recently reviewed a case that lead to a negative patient outcome due to poor staffing. 💫 Per ASPAN (American Society of PeriAnesthesia Nurses) standards for nursing staffing in the PACU (Post-Anesthesia Care Unit): "Two registered nurses, one of whom is a perianesthesia registered nurse competent in Phase I postanesthesia nursing, are in the same room/unit where the patient is receiving Phase I care. The Phase I registered nurse must have immediate access and direct line of sight when providing patient care. The second registered nurse should be able to directly hear a call for assistance and be immediately available to assist. These staffing recommendations should be maintained during “on call” situations." 💡 𝐓𝐡𝐞 𝐂𝐨𝐧𝐬𝐞𝐪𝐮𝐞𝐧𝐜𝐞𝐬 𝐨𝐟 𝐅𝐚𝐢𝐥𝐮𝐫𝐞 Unfortunately, the failure to maintain proper staffing levels in the PACU can have profound consequences. Reduced staffing may compromise patient safety, delay critical interventions, and hinder the overall quality of care. 👩⚕️ 👩⚕️ 𝐒𝐭𝐚𝐟𝐟𝐢𝐧𝐠 𝐑𝐚𝐭𝐢𝐨𝐬 𝐭𝐡𝐚𝐭 𝐏𝐫𝐢𝐨𝐫𝐢𝐭𝐢𝐳𝐞 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 𝐒𝐚𝐟𝐞𝐭𝐲: ASPAN recognizes the significance of maintaining appropriate nurse-to-patient ratios to ensure optimal patient safety. These standards are designed to prevent complications, promote timely interventions, and facilitate a smooth transition from anesthesia to recovery. 🤝 𝐂𝐨𝐥𝐥𝐚𝐛𝐨𝐫𝐚𝐭𝐢𝐯𝐞 𝐓𝐞𝐚𝐦 𝐀𝐩𝐩𝐫𝐨𝐚𝐜𝐡: The PACU is a dynamic environment that requires a collaborative team approach. ASPAN encourages a diverse team, including skilled nurses, anesthesia providers, and support staff, to work together. This collaborative effort ensures comprehensive care that addresses the unique needs of each patient. 🚩 In the case I reviewed, the hospital would often utilize one of the OR nurses as the second nurse. The PACU nurse would have to call into the OR if they needed assistance. In other words, the second nurse was not "immediately available to assist" per ASPAN standards. There were quite a few breaches of the standard of care which ultimately led to delays in patient care and a negative patient outcome. Want to learn more?
