The primary focus of nursing school is teaching us how to prevent harmful events with patients, and provide foundational knowledge for an extremely broad job field. An RN new grad is often referred to as a "baby nurse" for 2 years after school. The real learning of actually treating patients happens on the job. These two years are the most pivotal in a nurse's career. Surrounding a nurse with great preceptors is crucial to retention, engagement, and positive patient outcomes. When you have new people training new people as we often see today, we get the death spiral of burnout-quit-hire that has become a reality of our industry. Lightbulb Cardiac is breaking this cycle in the highly specialized world of invasive cardiology. We're providing expert preceptors to guide and coach new and experienced staff alike in the Cath and EP lab. No amount of textbooks or click-through modules can replace the on-site, personalized experience we provide our clients. Last Tuesday was our first combined hands-on competency review session for the ambulatory clinics at ECU Health Physicians, bringing together the nursing (RN, LPN) and allied health (CMA, RMA, MA, Rad Tech, CT) team members from academic and community primary care and several specialty clinics. This session allows team members to rotate through stations to review and validate high risk, low volume, and high impact skills. Many of us are used to the high risk/low volume skills but I added in a few items and referred to them as "high impact" skills. These are items that are frequently occurring in many areas (ex: blood pressure measurement) but if performed incorrectly it could have major impacts to the patient's treatment plan and the data that is submitted for hypertension control would not accurately reflect the patient's true disease control and the work our teams do each day. We have one more skills session coming up for our Greenville based primary care and community specialty clinics. Shout out to my team of two amazing nurse educators, Jennifer and Materria, who take this show on the road and cover all of our regional clinics with the support of the clinic staff and the support of Stacey and Kimberly Manning, the Quality Nurse Specialists within ECU Health Physicians. Huge thanks to our leadership who supported us in getting instructors and team members together for the day! I may be a little biased but this study from University of Pennsylvania School of Nursing showed that even a modest substitution in RN care is associated with poorer patient outcomes. There are multiple factors that go into this equation. Let's ultimately consider what is best for patients - not to mention that these findings are very costly to hospitals who are trying lower cost/waste. A 10-percentage point reduction in the proportion of RNs was associated with: 👉 7% higher odds of dying in the hospital 👉 Higher odds of readmission 👉 Longer length of stay 👉 Poorer patient satisfaction. https://lnkd.in/ek_pMqKx

Nurses are innovators--its part of our DNA. I remember very early in my career, I was working in a small community hospital and we had a crashing urosepsis patient. The patient couldn't pass urine and his urethra was malformed and could not accept a cath. There was no urology coverage and the patient was decompensating. After countless phone calls and consults our only options was to place a central line into his bladder to drain his puss filled bladder. We didn't have the right equipment but had to make something safely work--I mended a foley cath with some central line equipment to make a sealed system. It was not pretty--and I am sure it violated some safety rules. But again--we had to make something work as this patient's life depended on it. This is not an isolated story--and I am sure all of you have other stories of innovation in a pinch. I would love to hear about some of your best bedside innovations below. As I moved into a leadership role this ethos of innovation carried with me. One of the big pain points was BREAKS. Washington State has tight rules and windows when breaks need to be completed--balancing those rules with the chaos of emergency medicine is a logistic nightmare. So I set out working with a couple amazing software engineers to create Breakalgo. It is a dynamic nurse break management platform. It schedules, optimizes and tracks breaks. Breakalgo was selected by NurseApproved to be be part of a nursing innovation pavilion at ViVE 2024. There will be other great nursing organization like the American Nurses Association and the American Organization for Nursing Leadership that will be amplifying the voices of nurses at this amazing event.
