Frailty scores. 💥 how are you calculating them & how are they being used to create a patient focused care plan ? 💥 #FrailtyScores add value to the patient journey and can influence triage decisions. The challenge is that most patients with long term conditions don’t have frailty scores, if they do have one they aren’t shared easily with the clinical records in primary care and “frailty” as a concept is not something that most patients understand or recognise. I’ve had an interesting week. A Primary Care Network event around frailty on Wednesday afternoon generated some interesting discussion points and areas for development. The following day I was able to do a lift pitch at Redmoor Health Hackathon AI & LLM event on Thursday , which led to the idea being worked up a small group including patients & clinicians. With Dustyn Saint driving the ChatGPT licence we were able to create a decent model that achieved some of our aims and demonstrated what can be achieved in a couple of hours. I can see the benefit of having a frailty score recorded for all patients over a particular age or with long term conditions. These conversation about what frailty is, why it’s important and what can be done about it isn’t something that should be rushed or shoehorned into an already tight schedule. How could we do this differently ? Resulting in a better experience for the patient and a more relevant care plan that truly reflects the patients preferences ? We would like to be able to make it easier for patients , with support if needed from family or carers , to … 1. Generate their own frailty score without input from clinician 2. Get introduced to what frailty scores are and mean without it sounding off putting or unkind 3. Generate a care plan based around patients identified / or LLM suggested areas for change in different domains of wellness eg diet , exercise , social , balance , strength , medication , medical condition optimisation or pragmatic easing of medical targets 4. Once we have a frailty care plan we could send a copy to the GP notes. The Social prescriber could help facilitate any actions identified with the suport of the primary care network team. 🤔Questions ; has your organisation looked at how to automate the production of #frailty scores ? Are there any tools out there that already do this ? Specifically interested in ones which use language that is familiar with regular “non expert” patients ?
