I've been an RRT for over 13 years. The irony of my profession is: I am called when the ventilator is alarming, when "ish hits the fan", when the patient is decompensating. However, to get providers, especially NEWER providers to listen to me before the storm is like pulling teeth Lungs and ventilator is ALL I KNOW. it has been proven time and time again that respiratory driven protocols improve patient outcomes. So why are we still overlooked in the ICUs in so many hospitals? My only guess is for the same reason Ardsnet protocol: a proven approach to reducing mortality in pts with ARDS developed in the 80s wasn't taken seriously until COVID ... 😒. In this article of chest physicians, there's yet another study proving that respiratory driven protocols result in greater patient outcomes in SBT trials.

Updates to two respiratory assessment guides: Measuring peak expiratory flow https://lnkd.in/exbi9Ftf This guide describes how measuring PEF can be used alongside patient history, physical examination and other tests, such as quality-assured spirometry, to provide an accurate diagnosis of asthma. Once a diagnosis has been made, patients can use PEF to monitor their condition from home. This guide demonstrates how to instruct a patient on using a peak flow meter. The last page of the guide consists of a case study of using spirometry as a diagnostic tool in a person with suspected COPD. Competency checklists accompany both of these guides. Over the last two decades AARC has partnered with the Centers for Disease Control and Administration for Strategic Preparedness and Response to train clinicians on the role of the respiratory therapist in mass casualty care and the ventilators in the strategic national stockpile. All in anticipation of a mass respiratory failure event. COVID-19 tested this preparation and challenged the worlds health care system. Shortages of staff, PPE, ICU beds and ventilators strained even the most prepared institutions. Hospitals were required to improvise on the fly. Staffing models were created and the use of travelers became routine. The use of PPE amongst shortages placed caregivers at risk, challenging the duty to work paradigm. The role of the respiratory therapist evolved. New treatments were devised, tested and either adopted or abandoned. There were 3 types of ventilators in the stockpile (a total of approximately 14,000 ventilators). During the COVID pandemic, this number ballooned, with the addition of approximately 150,000 ventilators (over a dozen new models- 2 of the 3 original devices were scrapped). These challenges and the responses as well as future planning will be discussed in this symposium. Respiratory Therapists (RTs) utilized in the home have shown to reduce patient’s 30-, 60- and 90-day readmission rates for COPD flare ups. Communication and education between patients and RTs are key as comfort can drive compliance. The RT intervention at home focused on education, COPD action plan and follow-up.
