For example, when a veteran with #COPD struggled to get his wife, who uses a wheelchair, to her medical appointments, Clarion Hospital--a participant in the CMS Innovation Center’s Pennsylvania Rural Health Model--stepped in to help. A care transition navigator connected the couple to a program called Ramps for Hope, to build an outdoor wheelchair ramp so that the husband could get his wife safely out to the car by himself. The strongest possible affirmation of the whole thrust of this #AFR article. Our own experience over several admissions into acute care determined our mutual decision to do everything possible to allow for a gentle loving supported death at home. This did happen. Only possible through #palliativecare, #Districtnursing, #rostereddearfriends, #fulltimeCarerSpouse In no way underestimating the huge toll of such an approach. But it does and did make a good death possible. These are issues and approaches that deserve much more engagement. The clinical medicalisation of dying is not the only way.
