What are the true facility cost differences between an open and endoscopic carpal tunnel release (CTR)? Our recent time-driven activity-based costing (TDABC) analysis found endoscopic CTR to be 66% more costly to the facility. Endoscopic CTR was independently associated with a $350 increase in total facility costs, predominantly due to longer OR time (37 vs 25 min) and higher supply costs ($383 vs $110). To improve upon the value of endoscopic CTR, efforts to decrease OR time and negotiate endoscopic-specific supply costs are warranted. Upper limb Fractures & Orthopaedic Rehab initiation: The general management principles are the same for any trauma. A history including mechanism of injury, hand dominance, occupation and recreational hobbies, as well as current functional status are critical. A focused examination should always include neurovascular assessment of the affected limb, as well as assessing for open fractures, compartment syndrome if relevant, and any other bony injuries in the context of the polytrauma patient. Subsequent basic management should include splintage/immobilization and orthogonal radiographs in the first instance. Further imaging and or surgical stabilization will be covered in more detail hereafter. Emerging evidence says that current prescribed exercise regimens may not be effective in reducing impairments and improving activity following an upper limb fracture.
