This weekend, I encountered a patient whose goals were neglected, physicians weren't listening, and his sister was advocating tirelessly for him. In Value-Based Care, discussing patients' goals at every visit is paramount. In hospice, we adapt care plans daily to align with patient and family goals. Recognizing the importance of timely hospice care within the broader value-based care model is essential. As healthcare professionals, we must prioritize integrating hospice care into value-based models. This ensures that patients receive compassionate care tailored to their needs and preferences. 🏥💙 Bassett Healthcare Network, a New York-based health system, has partnered with Helios Care to provide patients with value-based hospice and palliative care. Patients in five counties across Central New York will benefit from the collaboration, which aims to establish a more cohesive continuum of care from birth to end of life. Helios Care currently serves three counties in Bassett Healthcare Network’s eight-county service area: Otsego, Delaware, and Schoharie Counties. Too many patients who would benefit from hospice move too late to fully benefit, or don't transition to hospice at all. Instead, they spend more time in home health than is advisable given their condition, resulting in insufficient care. Patients whose trajectory shows that they are approaching end-of-life care should be identified early to facilitate conversations with family and patients about hospice resources, compassionate care, and end-of-life planning.
