Collaboration between #primarycare providers and specialized #mentalhealth care providers allows services that were historically delivered separately to be delivered in the same setting and ensures a patient’s “whole-person” health needs are met during care delivery. The Collaborative Care Model, a team-based model of care delivery, is a promising integration model with a substantial evidence base. Undoubtedly, it's the automation of the #triage and #carenavigation process that sets us apart in the #primarycare access ecosystem.
We think we know the answer, but there’s always a nagging doubt there’s something we haven’t thought of. To escape the seemingly interminable demand / capacity crunch in primary care, adopting the Modern General Practice Access (MGPA) model, boosted by computer-based triage, seems to be the strongest approach. Looking at others in the ecosystem, our approach looks most radical (employing a digital care navigator that can assess and navigate thousands of patients per second). Patient Access Collaborative Founder & Executive Director, Elizabeth Woodcock, DrPH, MBA, FACMPE, CPC was the leader of an all-day retreat to kick off an access initiative for a large, multispecialty health system. The executive leader presented the focus on access as a critical component of the system's strategic plan; the co-chairs enthusiastically described their roles, and the participants formulated an action plan with priorities.
