Self-direction, also called consumer-direction, is a way for people who need long-term care to receive support and stay in their homes and communities. If you are in a program supported by Medicaid, you may qualify to be authorized by your state Medicaid agency for a certain amount of care at home. The state works with a Financial Management Services (FMS) provider to pay caregivers and support you in buying approved goods and services. The state also checks if you qualify for other types of support. Self-direction is about personal choice, control, and flexibility. In self-direction, you (or someone you trust to make choices for you) pick and train the people who care for you, plan when they’ll help, and choose how they’ll assist you. With self-directed services: - You stay in your home and community. - You pick your caregivers, possibly even friends or family. - You manage your care. You are in charge.

The number of children enrolled in ACA healthcare plans rose 40% between 2023 and 2024. While I wish I could celebrate, that still means over a million kids who lost coverage in the Medicaid unwinding remain uninsured. States must do more to keep kids from slipping through the cracks. One way to help children access care is to implement continuous enrollment for kids, which ensures they stay covered even when their parents’ financial circumstances may change. In California, Medi-Cal is a lifeline for many low-income families, with over half of children relying on it for healthcare. Yet, navigating its complexities, especially during renewal periods, can lead to eligible children losing coverage. During the unwinding process, over 330,000 children lost Medi-Cal coverage in California due to procedural issues like delayed renewal packets and long wait times for assistance.
