A recent publication in Medical Science Monitor addresses a significant shift in the understanding and diagnosis of fatty liver disease. The paper discusses the transition from non-alcoholic fatty liver disease (NAFLD) to metabolic dysfunction-associated fatty liver disease (MAFLD), emphasizing metabolic dysfunction's role in the disease's pathogenesis. Interestingly, a 2023 proposal for metabolic dysfunction-associated steatotic liver disease (MASLD) has not shown conceptual advantages and may perform worse than MAFLD criteria. This review provides critical insights into why MAFLD remains the preferred diagnostic framework. Research underway at Ontario's hospitals can help unlock the secrets of how treatments work and opportunities for advancement. Professor Daniel Drucker and his team at Sinai Health's Lunenfeld-Tanenbaum Research Institute have discovered a gut-brain-immune network that controls inflammation across the body and affects organ health.
The research centres on the effects of glucagon-like peptide-1 (GLP-1) receptor agonists, or activators, which clinicians use to treat type 2 diabetes and which have recently proven highly effective for weight loss.
“We know from clinical studies that GLP-1 does all this amazing stuff in people, but we don't fully know how it works," said Drucker.
Drucker has received some of the world's most prestigious awards in the life sciences for his many findings on GLP-1, including the 2023 VinFuture Emerging Innovation Prize and the 2023 Wolf Prize in Medicine. As well, GLP-1-based diabetes drugs that emerged from Drucker's early research were named 2023 breakthrough of the year by the Science Magazine.

Our partners from Health Research Institute of Santiago de Compostela and Servizo Galego de Saúde published their paper "The Hepatokine RBP4 Links Metabolic Diseases to Articular Inflammation" in the Antioxidants MDPI Journal.
The Muskuloskeletal Pathology Group, lead by Rodolfo Gomez Bahamonde,
investigated the role of retinol binding protein 4 (RBP4) in an articular context. They measured RBP4 levels and RBP4’s impact on articular cell types as well as performed proteomic analysis on primary human osteoarthritis chondrocytes, revealing relevant RBP4 concentrations in crystalline arthritis (CA) patients’ synovial fluids. These levels have also been linked to hypertensive patients with metabolic disruption. Furthermore, in their study evidence is provided that RBP4 acts as a DAMP at these concentrations, inducing robust inflammatory, catabolic, chemotactic, and metabolic responses in chondrocytes, synoviocytes, and neutrophils. These effects may explain RBP4-related metabolic diseases’ contribution to joint destruction in various rheumatic conditions like CA.

Our scientific co-founders Christian Ottmann and Luc Brunsveld, TU/e colleagues Emira Visser, Marloes Pennings, Peter Cossar, and academic collaborators at Mt. Sinai, Liora Katz, PhD, Donald Scott, Isabelle Tse, and the University of Duisburg-Essen, Markus Kaiser, Kathrin Plitzko, recently published on the potential application of a 14-3-3 molecular glue approach for diabetes: Molecular glues of the regulatory ChREBP/14-3-3 complex protect beta cells from glucolipotoxicity. This differentiated approach may provide a novel therapeutic strategy to address not only diabetes, but also other metabolic diseases such as nonalcoholic steatohepatitis (NASH) and fatty liver disease (NAFLD).

Precision medicine focuses on tailor-made healthcare decisions making according to individual characteristics. In complex diseases such as type 2 diabetes, where cardiovascular disease remains the leading cause of premature mortality, the identification of highly sensitive and specific prognostic biomarkers is essential to improve individual risk stratification and implement early intervention.
This systematic review, conducted on behalf of the ADA/EASD PMDI in support of the 2nd International Consensus Report on Precision Diabetes Medicine, reveals key findings:
- NT-proBNP, TnT, TyG index, and GRS-CHD score were the most promising markers for predicting cardiovascular disease risk in individuals with type 2 diabetes, with the strongest evidence for NT-proBNP.
- While some biomarkers and genetic scores show potential for improved prediction, there is insufficient evidence regarding their clinical utility for risk stratification, suggesting the need of further validation studies.

Excited to share that our latest research, part of the PMDI consortium, has been published in Nature Communications Medicine. As the first author (shared), I'm proud to have played a key role in this groundbreaking study. I feel honored to contribute to the Precision Medicine in Diabetes Initiative (PMDI), a global collaboration between the American Diabetes Association and the European Association for the Study of Diabetes.
Despite individuals with type 2 diabetes facing twice the risk of developing cardiovascular disease, predicting the most vulnerable remains a challenge for clinicians. Traditional risk scores are outdated and struggle to perform effectively in diverse populations. Our study, involving 23 experts from 11 countries, aimed to identify promising markers that could enhance cardiovascular risk prediction in individuals with type 2 diabetes.
This achievement underscores the power of international collaboration in advancing precision medicine, bringing us one step closer to more accurate risk assessment and personalized care for those with diabetes. Grateful for the opportunity to contribute to this impactful initiative!

My coauthors and I just recently published a review article in Cells on extracellular miRNA and its applications in acute inflammation, most notably in sepsis but also in ischemia/reperfusion injury. This is an interesting subtype of miRNA that was discovered in the late 2000s, and there is still much to be learned.
