Very sad! Disposal of chemical barrels without decontamination is an extremely wrong practice often ignored by process owners. In this case the barrel must have left over, that generated flammable vapour within a confined space creating explosive atmosphere. Such incidents are also very common with petroleum tankers on trucks when taken for repair works in workshops without decontaminating properly and work started without checking for presence of explosives. Many a times such disposed barrels are asked to be cut and made ready for some use in plants. Everytime the victim is the poor person who is totally unaware of the hazard and starts the hot work. Adding to the woes is the uncontrolled ignition source out of welding/ cutting spatters as can be seen in the video. I would personally assign the uncontrolled disposal of the decontaminated barrel as the root cause….. Taking learning from this, users must feel and take the responsibility towards this and dispose such barrels responsibly only after ensuring proper decontamination.
