Some anesthesiologists hope this is the year #Tennessee passes legislation to allow Certified Anesthesiologist Assistants to practice in the state. Assistants are authorized to work in 20 states but not in Tennessee. Sasi Duggirala, certified anesthesiologist assistant and managing partner at Harmony Anesthesia in Atlanta, a former Tennessean, said the state's shortage of anesthesiologists as well as Certified Registered Nurse Anesthetists delays surgeries. He is advocating for licensure of certified assistants to increase the number of anesthesia providers, saying it would ultimately improve patient care. "After coming out of COVID, there's been just a huge backlog on necessary procedures and surgeries that need to be done," Duggirala pointed out. "The mid-level anesthetist coming only from CRNA pool is just not enough to fill that gap." Last year, Senate Bill 453 would have allowed certified assistants to practice in Tennessee but the bill stalled in the House. Some Tennessee Certified Registered Nurse Anesthetists opposed it, claiming the assistants lack the comprehensive training of nurse anesthetists. Backers want to see the bill reintroduced.

The intuitive feeling of 'worry' in anesthesia practice serves as a catalyst for critical thinking, problem-solving, clinical reasoning, and decision-making. Nurse Lingo: Staff splitter – a patient who badmouths the previous shift’s staff, but praises the current staff Patient sundowns – (elderly) patient becomes confused and agitated in the evenings Patient needs 1:1 or patient has a sitter – patient needs close monitoring due to safety issues Frequent flyer – patient who comes to the hospital frequently Walkie-talkie – patient who is stable, who can walk and do activities independently LR or D5 half or D5 half with 20 of K – types of IV fluids for fluid replacement Code Brown -- patient had massive diarrhea Eating, voiding, ambulating – patient is meeting discharge criteria after surgery Is & Os --  intake and output – what a patient ate and drank, how much they put out Drainage vs. output—Drainage is usually wound drainage. Output is usually output from a drain, like a JP drain or Foley catheter. Offered emotional support—patient was having difficulty with the diagnosis, prognosis, hospital stay, family issues, or bills, and nurse offered support and reassurance. Vitals stable aka vital signs stable – no hemodynamic changes. Just means blood pressure, heart rate, respiratory rate, oxygen saturation, and temperature were within normal limits and did not change in the interval period. NAEO – no acute events overnight. Some nurses used to write just this in their shift summary years ago, but that practice has pretty much gone away. Meant that the patient was stable overnight; the nurse monitored the patient but did not have to contact the doctor or do any interventions. Can help with more if you tell me the phrase.

This includes monitoring vital signs, consciousness level, and pain management. - Documentation-Complete documentation of the patient’s condition, surgical details, and anesthesia used is essential. - Communication-Effective handoff communication to the receiving unit is crucial, ensuring continuity of care. Family Presence-Policies may vary regarding family access during recovery, typically requiring priorty 3. Emergency Protocols-Clear protocols should be in place for emergencies, including immediate access to resuscitation equipment. 4.Compliance with Regulations-All activities must comply with hospital policies, state regulations, and accreditation standards, such as those from The Joint Commission. Popular medical shows have given the public an image of the operating room and its reoccurring characters – the patient, the surgeon, the surgical nurse, and the anesthesiologist – but one character missing from those scenes is the niche role of anesthesia clinical assistant (ACA). ACAs support anesthesiologists in a variety of clinical settings and are specially trained to perform many of the tasks of an anesthesiologist, such as administer anesthesia, monitor vital signs, start arterial lines, and intubate patients. These tasks always occur under the supervision of an anesthesiologist who is immediately available. “Clinical assistants are physician extenders and are licensed by the College of Physicians and Surgeons of Manitoba,” said Jared Campbell, a veteran ACA who graduated from the University of Manitoba's (U of M) one-year ACA training program in 2009. “As medical staff, we are entrusted with a high level of responsibility. If I am in the operating room assisting one of the anesthesiologists and they have to leave the surgery to assess a patient in the emergency department or recovery room, I can stay in that operating room and continue supporting the surgery.
