Creating and sharing standardized electronic patient summaries( built on International patient summary standard) are foundational to improving continuity and transitions of care. And most importantly, it helps achieve our collective desired goal of providing patient centric care!!

Yesterday, I listened as a friend walked me through a lifetime of back-to-back heart-breaking stories regarding battles with their health, as well as some of their family members. Equally heart-wrenching was listening to the nearly insurmountable struggles they had to endure to piece together their care. Unfortunately, they could take me down their street and I could speak with neighbour after neighbour, each with their own heart-breaking struggles to tell as well. There were so many different takeaways from listening to this story that could fill ten posts. With such a long string of medical conditions they clearly had a very complex and interconnected healthcare situation. This also created a large list of medical professionals involved over a lifetime. Many of the conditions and the issues were on the rarer side. You can already feel for how this would be a recipe for struggle. They had to eventually resort to building their own physical medical record in a giant folder, kept at home. Photocopies brought to each new doctor to inform them as best they could. They had to strongly advocate for themselves, repeatedly calling, emailing, etc. to get the attention of the system, a system that’s clearly overwhelmed, but also not connected as it should be. Here we have a citizen who is trying to get care so they can live a productive and happy life. We also have a healthcare system with caring professionals trying to do their best with what they have to work with. Unfortunately, we have a major chasm that exists between both. Many things will need to be tackled to close this gap. However, one thing is for certain, the lack of a comprehensive, electronic health record that contained all of their past and current conditions, care, surgeries, tests, environmental exposures, treatments, medications etc. certainly played a key role in the struggles endured. I’ve mentioned before that I’ve been encouraged by the recent gains made here in NB with repect to electronic health records. More needs to be done. All practices, labs and healthcare centres etc will need to be properly connected to ensure data is appropriately, and securely being shared with our electronic health records. The data needs to be in a standard patient summary format to be usable. We need to help healthcare professionals ensure the data is captured but not overload them with more administration. We need their time focused on care. At a recent Canada Health Infoway conference, Dr. Rashaad Bhyat and Dr. Payal Agarwal discussed the patient summary format and why health systems should adopt the pan-Canadian version of it. Included is a fascinating live demo of how a patient summary can be easily captured during a patient interaction with the help of an AI assistant. This patient summary data can then better inform care delivery. There are standards and innovative technologies out there. How can we embrace them and how can we shrink the chasm?

It's all about being intentional and understanding how the people we serve prefer to communicate, and then using technology to meet those needs. When building an organization or company's strategic plan, technology becomes a valuable tool to reach our goals.
