So many health and social care providers are struggling with aspects of end of life and dementia care because of workforce shortages and a lack of investment in learning and development. We rely on CQC to flag these issues and to advocate for good quality palliative and end of life care for patients and their loved ones. The new CQC single assessment process introduces more evidence gathering from patients , families and professionals - if we are to eradicate poor quality end of life care we need to listen to all of their voices and stories. Going forwards The End of Life Partnership welcomes the introduction of partner feedback as part of the CQC single assessment framework. We see many examples of exemplary care, but unfortunately we also see reoccurring themes that hinder good quality care at the end of life. Let’s work together to instil trust back into the care regulatory system and to shine a light on both good and bad practice to ensure a strong a positive future for palliative and end of life care for all. #cqc #endoflifecare #palliative

This is at the core of improving care for all. By orienting our care to what matters to the individual, and truth telling about what to expect after this episode of care, we can steer our health system in a better direction.

I spoke with Jill AM MAICD, AM MAICD, from The Australian Financial Review about our death-denying culture and its impact on people, families, health and aged care systems. Put simply, today's systems aren’t serving us well enough. Australia’s public health systems are in crisis. Hospitals nationwide are overwhelmed, dealing with bed blocks, elective surgery delays, ED overcrowding, and ambulance ramping. A new evidence-based review from The Violet Initiative uncovered some startling truths: ✴ At least a quarter of all hospital beds are occupied by patients in their last year of life, predominantly elderly individuals. ✴ Elderly, frail people in the last stage of life often face a distressing cycle of hospitalisation following acute health deterioration. ✴ Nearly one-third of emergency calls involve patients nearing the end of life, most of whom are over 75. ✴ Typically, people are transported by ambulance from home or from residential aged care to the emergency department, admitted to the hospital, and often end up in intensive care on life support. ✴ Ironically, many of these older adults do not wish to be hospitalised and prefer not to die in a hospital setting. ✴ Research shows that dying patients often receive high-intensity care without clear benefits, frequently at odds with their known preferences. This issue affects everyone. I'm calling on federal and state governments to help all Australians talk about and plan for the last stage of life so more people can have the dignified and compassionate experience they want and deserve. Let's make the #laststageoflife the best it can be for everyone involved. Please share this message and don't hesitate to contact me if you'd like to help. Violet's CEO, Melissa Reader, will discuss this on 2GB later this afternoon. Tune in. Link to the article is in the comments. #healthcare #agedcare #compassion #support

With NHS trusts like University College London Hospitals NHS Foundation Trust adopting myAsthma, we aim to help to solve some of the key challenges highlighted in the #DarziReport, particularly point 5- highlighting the issues around waiting times and the strain on NHS services, from GP access to emergency care. Dr Kay Roy PhD FRCP, Consultant Respiratory Physician University College London Hospitals NHS Foundation Trust, commented; “We are thrilled to introduce myAsthma as a self-management tool to our community. It represents a significant step forward in empowering our patients with asthma to take control of their health. By providing them with personalised support, we believe this tool will greatly improve their quality of life. Additionally, the use of myAsthma in outpatient settings will help triage patients more effectively, ensuring they are seen in a timely manner and appropriately referred for the right investigations and services. Our team is excited to see the positive impact this will have on the asthma population across North Central London ICB."
