A properly trained professional understands the importance of labeling samples correctly at the bedside, so I’m not saying what he or she had done is right. Mistakes happen, and if a sample is not labeled, it's crucial to take another sample and ensure it is properly labeled. However, this issue is not solely the responsibility of nurses, if it was indeed a nurse 🤦♂️; all healthcare professionals can make mistakes. It's important to educate nurses, porters, and any staff involved in sample collection to prevent these errors before samples reach the lab. For example, just yesterday, a patient of mine experienced significant bleeding due to a misplaced CVC line. It took over 12 hours to discover the line was incorrectly placed. While it's essential to address these issues internally for educational purposes and to prevent future occurrences, it’s not appropriate to criticize any profession on a public platform like LinkedIn. Hence, I haven’t come up here, to specifically point out that a particular healthcare professional had done this or that. Everyone can make mistakes, and we should support each other and uphold the dignity of all healthcare professionals. Nurses are humans and we are not above mistakes, educate us and don’t criticise us publicly.

A nurse sent this patient sample without the patient label on the sample, but then called back 10 minutes later that she would tube down the label😂. Unfortunately, I can not process the sample in this condition. I need a redraw! The sample has been discarded. Patient safety is a collective effort. Although she was upset, I couldn't accommodate her request🤦♂️ Per CAP GEN. 40491, all specimens MUST be labeled in the presence of the patient at the time of collection. Unlabeled specimens will automatically be rejected. No EXCEPTIONS!

It would be interesting to ascertain why the sample was not labelled “in the presence of the patient”. Was the error the result of a knowledge gap, a “culture” issue, I.e., “this is how we always do it”? Are there barriers to completing the sample collection & labelling as one continuous action? Interruption may have been a contributing factor. Nurses are constantly interrupted during critical tasks like sample collection and medication administration. I worked as a nurse phlebotomist briefly and encountered fewer interruptions during sample collection than I did a “bed-side” nurse. I tap into my experiences as a front-line nurse as a Transfusion Nurse Clinican as they give me incredible insights into the challenges & barriers nurses face during sample collection labelling. Ask the five “whys.”

How do you prepare a wound care patient prior to their first hyperbaric treatment? Here are some guidelines as per the Wound Care Certification Study Guide 3rd edition: Patient preparation prior to first treatment a) Obtain a physician order that includes the indication for treatment, number of treatments, duration of each treatment, and air breaks, if utilized. b) Hyperbaric risk assessment should be completed by hyperbaric-trained nurse. c) Hyperbaric-specific instruction should be given. d) Consents to treat should be obtained.
