Healthy Lungs for Life (HLfL) is a global awareness-raising campaign established by the European Lung Foundation and the European Respiratory Society in 2014, prioritizing the importance of respiratory health. The campaign runs each year and is launched during the ERS International Congress at a host European city. A series of engaging activities, including air quality monitoring, distribution of “clean air route” maps, bike challenges, and mass exercise classes, are designed to encourage public engagement in the host city. This. During the hellish summer of 2021, India was particularly hard hit by COVID-19. Sitting safely in my isolation bubble in the US, with no flights available, I—like so many others—relentlessly worked social media and my clinician networks to secure oxygen cylinders and hospital beds for family and people I knew. Some died waiting for that oxygen cylinder or hospital bed. And yet, in the midst of despair and suffering, hope and action came from civil society in the form of shining lights like Hemkunt Foundation and American India Foundation (led by my wonderful friend Mathew Joseph). I don’t know how they did it, but Hemkunt found a way to deliver tens of thousands of oxygen cylinders for free—including to my immediate family and friends—all across Delhi and its satellite cities (home to about 30 million)—with no drama or desire for the limelight. Just basic human kindness. There was a trust system: use a cylinder, and return it for refilling. Resident welfare societies (India’s version of HOAs) played an important role. I remember Mathew’s Facebook posts with nonstop travel so AIF could deliver yet another oxygen facility to a community in need. In a pandemic, we need our community. And, we need oxygen. It shouldn’t be the domain of the haves, but the right of every single human. These organizations, among others, exemplified the best of humanity. I would love to see stakeholder network analyses supplement the positioning of hospital hardware. Yes, we need every hospital and clinic to have functioning oxygen supplies and equipment. And yet, there will simply not be enough hospital beds in LMICs in the medium term to meet the needs of another pandemic. So hub-and-spoke and mobile models of care are needed.
