We can’t even do this moving between two different primary care practices or moving between different parts of our health system. #internationalpatientsummary The secure Patient Summary functionalities, found in the province’s health information app, MyHealthNB, enables New Brunswick residents to cross both provincial and international borders equipped with a shareable, digital set of basic clinical data. This summarized version of a patient’s clinical data provides health professionals with the essential information needed at point of care, such as during medical emergencies, walk-in clinic visits, and transitions of care.

The scope for a shared care record is as simple or as ambitious as you want to make it. At the very least we could apply this here in Aotearoa to integrate primary / secondary care views of patient information. Last week at the TUANZ conference in the DHA workstream I heard yet again how frustrated primary care providers (GPs in this instance) are with not being able to view their patients secondary care record. It's as easy as onboarding a user can be (roles exist already for this use-case) leaving the complexity of active use (doing, not just viewing) to evolve as models of care are developed to support the shift-left strategy over time.

At Pilot we have worked with quite a few health innovators. One thing we've learned is that digital healthcare innovation can be a great thing for our public healthcare system when the focus is on improving patient experience. Instead of impersonal or corner cutting, it can mean more time with patients, less burned out doctors and more efficient referrals and diagnosis.
