CMS document explains why the Medicare Advantage Fees were not raised for Puerto Rico in 2025. It has a lot to do with the low utilization rate of the FFS (traditional Medicare) utilization in Puerto Rico, which is used as a benchmark. In particular to the fact that 13.9% of Medicare participants with traditional Medicare do not utilize it, compared to 6% in the rest of the United States. The FFS Medicare participants in Puerto Rico (traditional Medicare) are not a cross-section of most Medicare beneficiaries in Puerto Rico. Most Medicare beneficiaries in Puerto Rico were made to believe that Medicare was changing to Medicare Advantage and that traditional Medicare was being phased out. This misleading information was spread by the third-party salesmen working on commission. Most of the underserved Medicare beneficiaries chose to participate in a MA plan. Only the most educated beneficiaries understood the cost of losing their free selection. Now the MA plans in Puerto Rico receive 60% of the mainland fees, using the FFS utilization as a benchmark. MA plans are paying the price for their mischievous recruiting practices. The solution is to promote growth in the FFS MC segment in order to have their utilization approach the utilization of the Medicare Advantage participants.
