From the Becker's Hospital Review: some steps to help reduce hospital re-admission rates 🏥 One of the goals for hospitals is to lower re-hospitalization rates by ensuring patients have the proper education and understanding of their responsibilities, involvement, and steps to take after leaving the hospital. Those patients who interacted with higher engaged nurses were more likely to respond “Always” when asked about their care transitions, their needs being met, and understanding their discharge instructions. With increased understanding, patients feel more at ease and more likely to comply with their responsibilities, medications, and steps to take after leaving, leading to better health outcomes. #hospitalreadmissions #nurseengagement

Did you know that HCAHPS is changing? A proposed new version — effective in January 2025 — is up on HCAHPS online. Here is a snippet of some changes: New questions: During this hospital stay, how often were you able to get the rest you needed? During this hospital stay, did doctors, nurses and other hospital staff help you to rest and recover? Did doctors, nurses or other hospital staff work with you and your family or caregiver in making plans for your care after you left the hospital? Did doctors, nurses or other hospital staff give your family or caregiver enough information about what symptoms or health problems to watch for after you left the hospital? Here are a few that were removed: When I left the hospital, I had a good understanding of the things I was responsible for in managing my health. When I left the hospital, I clearly understood the purpose for taking each of my medications. Tell me what you think! Are you ready for these changes? Providing a great overall #patientexperience with solid communication will keep you ready for shifts like this. And it turns out that communication was a strong theme at the #patientexpeiencesymposium in Boston last week. More to come! with Patient Guide Solutions
