The RHEIA trial, presented at ESC Congress 2024, found that transcatheter aortic valve implantation (TAVI) is superior to surgical aortic valve replacement for women with severe symptomatic aortic stenosis. TAVI reduced the combined risk of death, stroke, and rehospitalization by 6.8% compared to surgery. It also led to fewer rehospitalizations and a shorter hospital stay, although it was associated with a higher rate of new pacemaker implantation and mild aortic regurgitation. This study investigated the outcomes of transcatheter aortic valve implantation (TAVI) for patients with pure aortic valve regurgitation using new-generation devices. It focused on a group of high-risk patients who were not suitable for surgery due to their severe condition. The use of #TAVI in these patients showed promising results, with a high technical success rate. Tranexamic acid / TXA is probably the most effective intervention for preventing bleeding in people undergoing hip or knee replacement surgery. Aprotinin and EACA may not be as effective as TXA at preventing the need for allogeneic blood transfusion. Unable to draw strong conclusions on the optimal dose, route and timing of administration of TXA. TXA given at higher doses tended to rank higher in the treatment hierarchy, more beneficial to use a mixed route of administration (oral and intra‐articular, oral and intravenous, or intravenous and intra‐articular). Oral administration may be as effective as intravenous administration of TXA. No evidence of harm associated with higher doses of tranexamic acid in the risk of DVT. https://lnkd.in/gp7f9-we

A 63 years old gentleman with known coronary artery disease (previous stenting and previous bypass surgery) presents with angina. The right coronary artery (RCA) was chronically occluded (A). Crossing was easy using an XT-A wire and for lesion preparation a cut & crack approach was used (Wolverine 3.0 @ 22 atm & OPN 3.0 @ 25 atm). Good luminal gain was achieved (B) and minimal luminal area on OCT was 4.0 mm2 with a deep dissection following a cutting balloon inflation. Two Selution drug eluting balloons (Sirolimus) were used (3.0 and 3.5 mm). 5 months later an excellent result was documented on coronary angiography (C). The dissection had completely healed resulting in an impressive luminal gain (4.0 mm2 --> 8.0 mm2). #dcb #luminalgain #drugcoatedballoon #lesionpreparation
