The "predictable surprises" in process safety. As we know, "foreseeable surprises" are all those issues that, through a careful system of hazard analysis, would have emerged and thus would not have produced surprise once materialized. How often in the field of safety do organizations "surprise" themselves? The problem is that new systems are complex and the "paths" of events are very difficult to "read" and "predict." How to ensure an effective security management system in complex systems? The perfect "recipe" for all systems does not exist. There are certainly a number of "pillars". 1) analysis with multidisciplinary teams and without "bias." 2) maintaining a sense of vulnerability. 3) maintenance of close monitoring in order to read weak signals and correct any deviation. ISO 31000 helps to set up a reliable risk management system, and applied with the methods found in ISO 31010 certainly chart a way forward. For the hazard analysis phase, the HAZOP and FMECA methods can certainly be successfully applied to understand: - what can fail, - in what manner, - with what consequences, - whether the organization is able to "see" it and avoid it. Giving PRIORITIES for intervention (through RPN). Prioritization, if given correctly, means planning interventions for improvement, giving time to do things well and with proper attention. There are no simple solutions to complex problems. But increasing the situational awareness of organizations is definitely the way to go. How do you demonstrate compliance❓ Although a lot of our process safety assessments take a risk based approach, there are many compliance driven activities that must be considered. We recommend you document these compliance commitments in a register which is regularly reviewed. It's also useful to explain which elements of legislation apply to your operation and how you comply. If you don't have this, how can you be sure you are meeting the regulatory requirements❓ This in turn may prevent the relevant controls in your risk assessment which compromises safety, not just compliance. We work with customers to create, review and maintain their safety management systems. We also support them with regular reviews and annual audits.
