World's First Robotic IST Ablation! In this video, Dr. Krauthammer and I discuss the world's first robotic Inappropriate Sinus Tachycardia (IST) ablation. Our patient, a 25-year-old female, had been suffering from persistent tachycardia, fatigue, and palpitations for years. We collaborated with AtriCure, Inc. to develop a novel robotic approach. Using a mitral valve approach with 4 ports, we ablated the lesion set with precision and control. The goal was to form scar tissue from the superior vena cava across the crista terminalis, avoiding the sinus node, down to the IVC, preventing inappropriate sinus tachycardia from escaping into the right atrium. The Epi-sense catheter helped reach challenging areas, and the robotic approach provided clear visualization and the ability to avoid dangerous areas. The advantages of the robot include 270-degree articulation, gentle tissue manipulation, and accurate contact with the ablation catheter, leading to greater control, precision, and potentially shorter procedure times. Transcatheter aortic valve replacement (TAVR) is a minimally invasive alternative to open-heart surgery to replace a narrowed aortic valve. Since next-day discharge (NDD) after the TAVR procedure has gained popularity in recent years, a team of researchers led by Wayne Batchelor, MD, Associate Director of Inova Schar Heart and Vascular and System Director of Interventional Cardiology, studied readmission rates and clinical outcomes in NDD versus non‐NDD transcatheter aortic valve replacement. After studying data from nearly 30,000 patients treated with uncomplicated self-expanding TAVR procedures, the researchers concluded that NDD should be the default strategy for patients undergoing uncomplicated TAVR procedures since it has improved patient outcomes, making a significant advancement in cardiac care. Philips has announced FDA approval for an enhanced version of its LumiGuide endovascular navigation wire. The new 160cm FDA-approved LumiGuide guidewire utilizes Philips' Fiber Optic RealShape (FORS) technology and was first used by vascular surgeon Dr. Carlos Timaran during a complex aortic aneurysm repair, marking the 1000th patient treated with FORS technology since 2020. This longer wire enables US clinicians to visualize a broader range of catheters and expands usage to more patients and procedures. LumiGuide allows doctors to see guidewires and catheters in 3D and color in real-time, with minimal radiation, unlike traditional X-ray fluoroscopy. Dr. Atul Gupta, Chief Medical Officer at Philips, highlighted that LumiGuide improves workflows, reduces procedure times, and decreases radiation exposure.
