Hyperpigmentation can be a frustrating outcome, especially for patients with darker skin tones, like my patient Fei, who developed pigmentation after a treatment at another medspa. It's so important to consult with an experienced physician who understands how to safely treat skin of all types. For certain skin types, like Asian and other darker tones, pre-treating with topicals is often necessary to minimize the risk of hyperpigmentation. In Fei’s case, a combination of topicals and laser treatments is key to achieving the best results. What should you do if you have a patient with an ACL injury that you suspect may be suitable for the Cross bracing protocol? If you have a patient with an ACL injury that you think may be suitable for Cross bracing protocol, then: 1. Advise the patient to avoid NSAID’s. Use Paracetamol for pain 2. Advise the patient to try to keep the knee bent (30 to 90°) 3. Avoid putting a brace on, especially a Zimmer splint 4. Here is a new case by Dr. Renee Genova, MD, FAAOS and Lakeland Regional Health-Florida Health. SPIRAL DISTAL TIBIA FRACTURE IN 84M HPI: An 84-year-old male with atrial fibrillation on Eliquis who is ambulatory with a walker sustained a right closed distal tibia and fibula fractures. The patient was initially treated with external fixation for soft tissue swelling. The patient has a long-term history of moderate stasis dermatitis and presents for definitive fixation. PMH: Atrial Fibrillation, CAD, history of dermatitis in bilateral lower extremities PE: The patient appears to be his stated age. The right lower extremity is swollen, and there is bruising throughout it. The skin is intact. He has a palpable DP/PT. He is able to fire his ankle and toe flexors and extensors. Sensation is grossly intact throughout the extremity. How would you manage this patient?
