Thank you Greater Ohio Vascular Access Network GOVAN for shifting the paradigm! It was nice to present on the adult patient perspective of living with long-term vascular access devices alongside, Tara, who is a PICU nurse and a parent to her son with TPN, and Mickey Hawes, nurse, researcher, consultant, previous Home Infusion Company exec., and PICC patient. The presentation was titled, “The Story Behind the Line: The Journey You Don’t See.” Todd Heslep BSN, RN, Paramedic, VA-BC, President, and the Executive Team at GOVAN made sure a patient and family perspective was shared in a meaningful way at their network meeting by: 1. Having us as the first speakers for the day. Doing so respects our time as the rest of the content is catered to clinicians and allows us to speak to audience members after having shared the bulk of our story so we do not have to repeat or experience any repeated trauma in resharing. Starting with the patient and family perspective can help set the desired tone and provides a good reminder of our why. 2. Valuing our time and energy as individuals and as a group. 3. Giving us autonomy in our slides and our call to action to the audience. I also thought putting questions from the audience in the middle of the presentation changed up the style and pace nicely and allowed the audience to feel included and have their questions answered. Great questions were asked pushing us to a good panel discussion. It was a joy to present and be a voice alongside Tara and Mickey’s thoughtful and direct insights. We shared the education we received to care for our lines in the home that has been most useful and the hardest to implement, and tips on how to communicate with us as adults, children, and caregivers, in the hospital at different points of access and the resources we may or may not have in the home. We truly asked everyone to understand this is our life not our job and we want you to take ownership of care delivery WITH us. We must work as partners. Being in a more intimate audience I got to see more closely for the first time clinician reactions of shaking heads, jaw drops and big eyes, when they learn I have had 31 central lines, 26 CLABSIs, 15 yrs no infection, same line for >5 yrs, and too many sticks w/o ultrasound. This is my why. This is why I continue to educate and travel to present. I get similar reactions in the hospital as an adult, especially at non-subspecialty hospitals stating I am the problem instead of asking why I had infections and line placements, what has been and is currently working well, and how can we continue that for you in this hospitalization and moving forward beyond these 4 walls. No one else should go through what I have gone through. We all have to learn from mine and the larger patient and family community experience for the next generation, for my gutsy peers, and for my friends who I have not met yet. #vascularaccess #vascularhealth #centralvenouscatheter #rarediseases #isavemyline

Through a Google search to understand more about todays article in The Times on NHS staffing budgets I instead found this, from 2016. It’s just incredible to read the themes then and now: ➡️ In 2016 we were relying on EU nurses coming to plug the gap, with a suggestion of a cap of 3-5,000 a year (there were over 23,000 nurses who joined internationally 21/22) ➡️ It was cited that “was no good reason why the supply of nurses cannot be sourced domestically”, adding that there was presumption by the NHS and government that non-EU nurses provided the NHS with a “get out of jail free card”. ➡️ There are complex institutional structures, which blur the decision making process and lead, amongst other things, to poor information and data making it difficult for them (and us) to understand and respond meaningfully to labour shortages ➡️ A lack of lessons learnt from the late 1990s-early 2000s when a similar shortage (and reliance on foreign nurses) occurred. ➡️ Almost all issues relate to, and are caused by, a desire to save money which ultimately result in increased costs and reduced quality services 8 years on were in an even worse place and the headlines are to spend less on Doctors and Nurses… 🤦♀️

CHESTERFIELD COUNTY, VA - After more than two years of construction, a nine-figure expansion project at St. Francis Medical Center is coming to fruition. Bon Secours is largely finished with its project to add a two-story, 110,000-square-foot addition to the hospital near Midlothian. The $108 million project included 55 new acute-care beds, as well as renovations to 70,000 square feet of existing space and other new equipment. It brings St. Francis to about 463,000 square feet in size. The additional acute-care beds are anticipated to be fully operational by early June, according to St. Francis COO Joey Trapani. He said they will bring the hospital’s acute-care bed count to 185. Also included in the project is the addition of 10 new observation beds, as well as four more beds in the neonatal intensive care unit for a total of 14 beds in that department. A new MRI machine, a new outpatient pharmacy, and expanded intensive care unit were also part of the additions. The project is the largest to take place at St. Francis since it opened in 2005, and was pursued by the health system to keep up with the demands that come with a growing population in Chesterfield County, St. Francis President Joe Wilkins said. “At that point in time there wasn’t much out here. But now, as we know, the Hull Street corridor is expanding,” Wilkins said. “There’s an influx of people that are moving here. We want to make sure we have the correct services to take care of those people.” Wilkins said the St. Francis emergency room, along with its two affiliated free-standing emergency centers near Westchester Commons in Midlothian and in Chester, have seen an increase in visits in the last several years and that a notable subset of those patients end up in need of hospitalization. “We have seen a tremendous increase in ED visits from those three ERs, and on average about 15% to 18% of those patients need admission to a hospital bed. So when you have an increase in those ED patients, and those patients need a place to be admitted, we need to have capacity to take care of them,” Wilkins said. The three emergency departments reported 6,342 visits in April 2024, compared to 2,242 visits in May 2022, which is the year the Chester ER opened. Wilkins said the Chester facility had about 28,000 patient visits in 2023, making it the busiest free-standing emergency center across Bon Secours Mercy Health, which is the U.S. division of the Ireland-based health system. To staff the additional space and meet demand, the hospital plans to add 100 more employees to its ranks. Bon Secours said an unspecified number of positions have been filled and that hiring was still ongoing. Do we really need to alleviate ED overcrowding or just to complain about it? If so, there is a way to do so. For many years, the issue of ED overcrowding/boarding plagues our healthcare delivery system. It has been exacerbated during and after the COVID pandemic for well-known and already intensively discussed reasons. Hundreds (if not thousands) of professional publications have been written. Myriads of media coverage addressing the importance of this issue have provided examples of the terrible consequences for patient mortality and medical errors from ignoring this issue. This problem has been significantly worsened during the COVID pandemic and by the increased patient demand due to the baby boomers. This not just the US’s problem. It is a burning issue in Canada, Europe, Asia and Africa. In fact, in some countries, EDs are periodically closed due to the imbalance between supply and patient demand, and patients have to drive to the next open ED which could be located a hundred miles away. A couple years ago, a group of the US ED chairmen published a very informative paper: https://lnkd.in/d2kFVr6j “The impact of ED crowding on morbidity, mortality, medical error, staff burnout, and excessive cost is well documented but remains largely underappreciated”, where they analyzed the most popular interventions on reducing ED overcrowding. At the top of this list is smoothing elective (mostly surgical) hospital admissions (see table 2). Indeed, as shown on the first comment on this post, unsmoothed hospital admissions adversely impact quality of care, staff burnout and hospital bottom line (https://lnkd.in/dvvubGN): It has also been proven to significantly improve nurse staffing, retention and access to timely surgery: https://lnkd.in/eB7Aw4xA. A week ago, Annals of Emergency Medicine published ahead of print a manuscript by my distinguished colleagues Gillian Schmitz, MD, Peter Viccellio , MD, and I “Emergency department crowding after Coronavirus disease 2019: time to change the hospital paradigm”: https://lnkd.in/gqUECQsq. It ends with the following words: “For the sake of a huge population of patients awaiting definitive care and for those who need critical emergency care, we need to change hospital paradigm.
