Time Sensitive! 🚨 Advocating for Change: The Three-Night Hospital Admission Rule 🚨 “Protect Patients and Support The Improving Access to Medicare Coverage Act” Every day, individuals across the nation are denied access to the post-acute care they desperately need due to the current three-night hospital admission stay requirement for Medicare coverage. The current policy requires a patient to spend at least three consecutive, qualified admission nights in a hospital before they have the option to choose Medicare-covered post-acute care to help them recover. The catch is that days spent in hospital observation status are currently NOT counted towards these nights often leaving people in a state of crisis once discharged. Having been with patients and their families at their bedside for over 15 years, I have witnessed firsthand the negative effects of this rule. When Medicare criteria keeps patients in the hospital to monitor and treat their symptoms under an observation status, the sick and injured often continue to lose their previous level of functioning. The inability to choose Skilled Nursing and Rehabilitation often referred to as Subacute Rehab as an option covered by their Medicare benefits often results in increased health risks and unnecessary financial burdens on patients and their families. This current rule leaves many in crisis and often perpetuates a hospital readmission cycle. It’s time for a change! We need your help to advocate for the elimination of this restrictive rule. If the legislature can be extended and enough use cases created to prove the concept so that all Medicare Beneficiaries have access to the "hospital-at-home" model beyond 2027, then not only will the cost of care tremendously decrease at hospitals, but it will provide significant solutions to overcrowded hospitals, increase reach and access to rural areas, decrease hospital readmissions, and improve patient's overall quality of life. Yes, there's still a lot to figure out, primarily the billing concepts, as this is not directly a traditional Medicare Part A Service; perhaps the MLTCs can pick up on this? As someone who had spent much time caring for an ailing family member in their final transition, I have seen tremendous benefits of this concept - primarily making their final time easier, kinder, and more humane - and it gave our family something that can never be taken for granted, seeing more smiles and peace on the family member's face when they were at home - rather than the agony, fear, contempt, and delirium when they were in the hospital. A new #AgeFriendly #Hospital Measure adopted by the Centers for Medicare & Medicaid Services (#CMS) for 2025 will promote high-quality care for older adults. Taking effect in 2025, the new quality measure builds on the success of the Age-Friendly Health Systems movement, which popularized the 4Ms Framework (What Matters, Medication, Mentation, Mobility). “This structural measure seeks to ensure that hospitals are reliably implementing the “4 M’s”, and thus providing evidence-based elements of high-quality care for all older adults.” (CMS final rule, page 1426). Johns Hopkins Activity and Mobility Promotion tools 🧰 have been included in both IHI Age-Friendly guides and American Geriatrics Society published white papers.
