Explore the Medicaid 3% SDoH waiver from the perspective of the delivery team members and what this does to them Remember that they are particularly overwhelmed with fewer team members, lesser team members or both 1. due to profit focus from the large employers taking over 2. due to survival focus where the population has worst public and privaet health plan concentrations See what this does to Community Health Centers with 50% Medicaid patients. 1. It is a 3% cut in Medicaid funding to support care delivery 2. It requires team members to move outside of encounters and take action before and after encounters to arrange resources, make contact, and probably attend more meetings. Follow ups are also added regarding food, housing... 3. The encounters are longer and more complex 4. Do we really see SDoH as just food or housing instead of dozens of factors shaped over decades of previous life experiences or generations of poverty? We should not let poor assumptions dominate health care designs, especially regarding more for overburdened delivery team members to do.
Attention Medical Associations: The Physicians Foundation is offering a grant of up to $150,000 for proposals to help physicians understand #driversofhealth (DOH) and integrate into their #medicalpractices. This grant program aims to spread awareness and knowledge around the best strategies for physicians who care for patients affected by DOH. Medical associations, closely aligned with physicians and patients in their communities, can provide critical insights to customize DOH screening and resource navigation within medical practices. Corporate Strategy Quarter 1 Progress Update When we launched our new Corporate Strategy earlier this year, we committed to keeping our patients, community, partners and staff updated on how we’re performing against our five strategic priorities (and the specific deliverables within each) we agreed for this year. As a reminder: 1. Our promise to our community (our vision) is looking after our community and providing excellent care. 2. Our values are: • Open and honest • Respect • Kindness 3. Our Strategic Objectives are: People, Patient Safety, Performance, Potential and Place. This is our first quarterly update which we discussed at our public Board meeting earlier today. How we did in Quarter 1: There are 77 deliverables in our Corporate Strategy Delivery Plan 2024/25 in Quarter 1. At the end of June, 3 actions were rated red (significantly delayed), 6 actions (partially delayed) and 68 actions green and are on track.
