Take ownership of the person living with dementia’s medical history! The first step in this process is gathering and organizing this history for ourselves. We use, but do NOT rely on the computer. I sometimes get pushback on this, but then clients tell me that this exercise opened their eyes. This process maximizes their time with healthcare professionals and reduces feeling caught off guard or guilty when choices must be made. It’s the way to start seeing into the future and coming to terms with the most likely decisions that will need to be made. It helps them take back some control from dementia. As part of my "Make Your Plan" course, I offer personalized support to every participant. I review their person’s organized medical history—after guiding them through what is relevant to gather. Together, we get clear on the decisions they’re likely to face, including treatment options, medications to watch, necessary services and resources they need to understand. If there are specific needs not addressed in the course, I provide that guidance via email as well. My mission is to help them become informed with a plan to fall back on so they feel confident and at peace speaking for their person.
