I'm interested in what can be done with electric stimulation. Abbott recently got approval for their implantable Spinal Cord Stimulation (SCS) device. Use cases are back pain and diabetic neuropathy not responsive to more conservative treatments. I could also see this being used for sciatica. A few considerations: (1) Specifically for diabetic neuropathy: Diabetic neuropathy leads to a degradation of the peripheral nerves. It is unclear to me this would help long term, as this does not address the root cause of the disease. (2) From an economics perspective, practices could potentially use CPT codes for remote therapeutic monitoring (RTM), which applies to MSK pain. Overall interesting tech. What are we stimulating when placing the lead through the foramen? Most likely, we are stimulating the root as a #PNS. However, in classical spinal nerve root stimulation, leads are mostly in close proximity to the DRG. Our recent publication highlights not just one case, but the first case of an extraforaminal approach (called #Xtra4) for stimulation of the distal nerve root as a PNS in #CRPS of the lower limb. Results of the most recent assessment of this patient are stable, similar to the fourth assessment in the paper, achieving long-term successful pain relief to date. This patient is the first of many who have benefited from the Xtra4 approach to SNRS since then. #SNRS can be a viable option, is safe, and in our practice, is indicated as a first-line treatment for focal neuropathies. #Xtra4 approach is a good alternative in cases when transforaminal placement is not feasible.

Auricular vagus nerve stimulation (aVNS) could be an effective non-drug therapy in the multimodal treatment of chronic and acute pain.
