“Occasionally, we are asked why patients undergoing a surgical procedure cannot act as their own controls in a clinical study. After all, prior to their procedure they exhibit a certain level of pain and disability, and after the procedure there is a change; often they have less pain and less disability. Is this not the result of the procedure? The short answer to the question whether the change is due to the procedure is simply, “maybe, maybe not.” While it is true that the change occurred when the surgical procedure was given, the change is not necessarily the effect of the surgical procedure itself. How can this be? The answer is that the treatment response is about more than the surgical procedure. It is about the cultural meaning of treatment: the way the surgery is proposed by the surgeon, the ceremony around the procedure, and the expectations of the surgeon and patient. I am often asked: How do you treat intersphincteric fistula? That is a relevant question as 1/3 of my fistula practice deals with referred patients failing muscle sparing surgery for IS fistula-MSS. In effect, MSS (Laser, VAAFT, LIFT,etc.) was devised for transphincteric and complex fistula, and not for IS fistula. There are two aspects of Intersphincteric fistula that need consideration: 1)The tract is less defined and when the external orifice is at the level of IS space (true one), MSS becomes merely a debridement of intersphincteric space and closure of internal orifice. Inducing coagulative necrosis in the IS space may actually open up space with higher chance of injuring the 4 mm IAS. 2) When the external orifice is lateral to the intersphincteric position, the tract may be more mature as crossing the corrugators/subcutaneous portion of EAS (false IS fistula). Results of MSS for IS fistula in literature are very scant, but higher failure rate is reported and expected. What do I do? If internal orifice is at or below dentate line: fistulotomy. GMC response regarding non-doctors making FUE incisions >> It is the position of the BAHRS that, in the UK currently, only GMC registered doctors should make incisions in the skin during hair transplant surgery which includes the skin incisions made for the Follicular Unit Excision (FUE) method of donor hair harvesting. An enquiry to the General Medical Council on this issue received the response that refers to the CPSA Hair Transplant Surgery Standard which clarifies on page 13 and 14 the roles of Hair Transplant Surgical Assistants. In this standard, it is explicitly stated that these non-doctor assistants must NOT perform the surgical steps of the procedure including making Follicular Unit Excision (FUE) incisions.
