Based on assessment information with the client develops and initiates the Community Living Plan, which is client-centered, comprehensive and consistent with program guidelines and policies and procedures.Conducts home visits, acute hospital & skilled nursing facility visits, as well as escorts clients to medical and other appointments as clinically indicated.Identifies, arranges for, and monitors appropriate community services based on a good knowledge of Medicare, Medi-Cal, and other entitlement programs.Establishes and maintains a care management relationship with clients and their informal support network as appropriate, offering respect, dignity and support. Provides crisis intervention, advocacy, problem solving and therapeutic interventions.Meets with clients at least monthly, and more often as clinically indicated. Reviews and modifies their Community Living Plan on an ongoing basis.Documents via progress notes all case management activity regarding identified problems within 24-48 hours, adding any new problems to the Community Living Plan, as needed.Maintains required paperwork and follows a clear, concise, and consistent system of charting to allow for continuity of care.Ongoing evaluation for client Purchase of Service needs and follow-up to determine if services have been provided in a timely manner.Educates clients and informal support network about resources.Establishes and maintains open and effective communication with community providers, including physicians and other health care and social service workers. Provides appropriate information on all significant aspects of individual client care and program operations, while maintaining necessary confidentiality.Monitors the quantity and quality of the services

Are we not already seeing this on an increasing scale anyway? Super-partnerships and national providers are becoming ever-more common, so I'm failing to see the groundbreaking wisdom of recommending something that is already on the increase. The caveat that it's not mandatory though is key.. this would never work in rural areas and it fails to take into account differences in culture, approach, values, ethos etc. Super-partnerships work when you have a collective set of values, but some practices are just better of working independently... We had productive conversations with four House and Senate offices today as part of AHCA/NCAL’s Congressional Briefing. I appreciate our elected officials from Minnesota and Colorado for taking time for these important discussions. We had the opportunity to highlight how we're focused on our employee experience at the Good Samaritan Society, and shared that retention is one of our top priorities this year. We strive to create the best possible culture in our locations, and having visited dozens, I feel that sense of community immediately when walking into a building. We shared in our visits that providing a great employee experience is an important component of addressing long-term care workforce challenges, but it does not change the reality that there are not enough caregivers available to hire. This is true today and will get worse in 2-3 years when the Administration’s new staffing requirements go into effect. Only 5% of our locations meet the requirement to have an RN onsite 24 hours a day, and it’s impossible to imagine where those RNs will come from, especially in rural areas. We need common sense, forward-looking approaches to support resident care needs while protecting access to care. We discussed immigration, virtual care, caregiver training and other policy solutions and investments that will be more effective in ensuring seniors have access to high-quality care.
