Analysis of five HTAs of medical technologies in 2023 and strategic recommendations for evidence generation strategy in England. Health technology assessment plays an important role in market access for medical technologies in England. National Institute for Health and Care Excellence (NICE) is the key HTA organization with several programs dedicated to the assessment of medical technologies. One of the key med tech-related programs at NICE is the Interventional Procedures Program. The evidence requirements in the program are not entirely clear and depend on many factors (population, level of unmet need, level of invasiveness, safety profile, and many others). MTRC developed an analytical White Paper with review of five recent assessments by NICE to present the evidence requirements for medical technologies in this particular HTA program. A review of five cases is followed by recommendations in relation to evidence generation for medical technologies, market access in England, and factors influencing the decision-making of NICE. The under-recruitment of patients into clinical trials creates a significant barrier to translating basic science discoveries into medical practice. This has a direct knock-on impact on the treatment and quality of care that patients can receive. Primary care data could help to tackle this issue. The richness and continuity of the primary care records means that it can be easier to identify patients for clinical research than it would be using other methods. What’s more, GPs agree - 81% surveyed think that primary care has a significant role to play in furthering clinical research for patient benefit. So why are less than half of GP practices participating? The latest report from Newmarket Strategy explores the challenges of engaging primary care in clinical research delivery and puts forward six recommendations for improvement. Personalized Medicine: Advances in clinical science contribute to the growing field of personalized medicine, tailoring treatments to individual patient profiles for more effective care. **N-of-1 Trials: The Future of Personalized Medicine** As a clinician deeply immersed in the realms of lifestyle and longevity medicine, I often contemplate the evolving landscape of clinical research. The challenge we face is significant: how do we replicate the findings of broad clinical research in our own, uniquely varied patients? The Limitations of Traditional Clinical Research In our field, every patient is a distinct entity, with their own genetic, biochemical, and environmental makeup. This diversity is the bedrock of our practice, making the replication of generalized clinical trials not just challenging, but often unfeasible. Why I Advocate for N-of-1 Trials This is where my passion for N-of-1 trials comes into play. These trials represent not just a method, but a paradigm shift in clinical practice. In these trials, the patient in front of us becomes the ‘experiment’, their own health data driving the treatment decisions. 1. Tailored Treatment Decisions: Using their unique health data, we can discern what works for THEM and what does not, ensuring the treatment is as individual as their DNA. 2. A New Clinical Standard: With medical technology advancing rapidly, I foresee a future where N-of-1 trials surpass the current gold standard of double-blind RCTs in precision medicine. 3. Learning from Every Patient: Each patient teaches us something new. This continuous learning ensures that we, as clinicians, are constantly at the top of our game, providing care that truly resonates with the individual’s needs. The Future Is Personalized I predict that the one-size-fits-all approach in clinical research will soon become obsolete. N-of-1 trials are set to be the recommended practice, revolutionizing how we approach patient care. As the medical field continues to evolve, so must we. By embracing these changes and advocating for more personalized treatment methods, we can ensure that our practice remains at the forefront of medical innovation.
