Love this. One of my most cherished duties is writing the “Why Hospice, Why Now?” letter to the team after my individual and family assessment is complete. Storytelling is a major part of hospice and I love that people trust me to share their stories with the medical team as they enter into hospice services. It also further helps me advocate for un-addressed needs for the IDG team. Before a single provider walks through the door of one of our patients, they will have a complete picture of every aspect of this person’s life. #advocacy
