What started as an idea 💡 a few months back,to seing it come to fruition by creating a split rotation between London North West Healthcare NHS Trust and NHS London Procurement Partnership has been great! Are you about to conduct a Pharmacy Monitoring Visit? Yay, fun! 🙌 The reason I loved pharmacy visits was because, as opposed to the regular monitoring visits or site management from the office or home, when I was at pharmacy I was working with physical objects (drug kits) instead of paper records or electronic documents. 🤲 But how do you conduct a Pharmacy Monitoring Visit? 👉 Check the Pharmacy Delegation of Authority Log: there might be a log mentioning who the main pharmacists is and every member of their team. A Delegation of Authority log that later on, will need to be integrated wit the main site log. 👉 Check the Drug Accountability Log: count, count, count all the movements of the study drug. How many kits came in, how many were dispensed, how many were returned. Not only kits but also tablets (if tablets). That’s how you’ll know patient’s adherence to the treatment and compliance. 👉 Check the storage (fridges) and the physical kits: count again all the kits left in storage and ensure the temperature of the storage facility is according to the recommended temperature for storage mentioned on the kit. 👉 Check the temperature records: All kits arrived at site come with a temperature log to know, every minute of the transport what the temperature of storage was. If any finding you will have to report it as Temperature Excursion. 👉 Review the training logs of the pharmacy staff: review and confirm all pharmacy staff members are trained and trainings are recorded in the training logs. 👉 Monitor the Pharmacy site file (some pharmacies keep their own binder with source documents): Are all versions of the Pharmacy Manual filed? Is the latest version filed? Are all accountability logs up to date, etc. 👉 Monitor if there are any preparation or prescription sheets signed by the invesigator or the pharmacist. #clinicalresearchassociate #clinicaltrials #clinicalresearch #pharmacymonitoring
