The 2023 Legislature passed an MMA priority directing the Minnesota commissioner of health to assemble an advisory committee of stakeholders to study and recommend how to implement a statewide registry for Provider Orders for Life-Sustaining Treatment (POLST) forms. That advisory committee finalized their report and recommendations in the fall of 2023. The MMA is now pursuing legislation to implement those recommendations. On February 22, Sen. Judy Seeberger (DFL – Afton) and Rep. John D. Huot (DFL – Rosemount), introduced SF 4154 and HF 4063. The legislation creates a statewide electronic registry of POLST forms available to EMS providers, emergency physicians and other appropriate providers to ensure patients’ medical treatments and end-of-life preferences are followed by all healthcare providers. Seeberger and Huot are both EMS providers. Currently, physical paper copies of a patient’s POLST form are given to the patient before transferring. However, these copies can get damaged, misplaced, or become otherwise inaccessible. This legislation would ensure a patient’s POLST form is electronically accessible by caregivers and EMS at any time and from anywhere. #patientcare #healthcareproviders

Show me some progress for 40% of the population in 2621 counties lowest in health care workforce forever with just 25% of the primary care workforce paid less to the tune of less than 20% of primary care spending. This works out to less than 50 billion. Show me at least 10 billion more on the way to the 100 billion plus needed so that these practices can have sufficient team members and health professionals to integrate, coordinate, and do patient centered are. I want proof of funding changes - no promises, no articles, no reports Don't care if this goes to CHCS, small practices, medium size practices, whatever - but anything less is a violation of CMS guidelines, health equity, and Belmont principles that we must apply to health care design the same as with physicians and human subject researchers.

The Medicare Program, one of the cornerstones of our healthcare system continues to underpay both hospitals and physician practices. Physician practices are on the brink of a cut, unless Congress intervenes. The Medicare fee schedule has never kept pace with inflation, and hospitals are in grave danger as well. The following are goals that the healthcare system in the US should work on ASAP: 1. Lower cost: there are too many middlemen extracting billions from the system 2. Increase quality of care. 3. Increase patient safety. 4. Patients deserve a better experience in navigating the healthcare system. They deserve more time with physicians, and they need their own primary care physician. They need to be treated with respect and have much better access. 5. We need transparency of all costs. These goals are a starting point, and I have no doubt that others will add valid points. Physicians are increasingly employed and have less control of the system daily. I don't put this list up to insult anyone but merely to generate a positive discussion. Restoring the primacy of the physician-patient relationship would solve many of these issues. Thank you, and I hope everyone has a safe and happy Fourth of July.
