Daily Critical Care Pearls #80: High-Dose Nitroglycerin for Acute Heart Failure Syndrome 📌 Most patients who present with Acute Heart Failure Syndrome have both increased preload and increased afterload which contribute to decreased cardiac output. 📌 Nitroglycerin reduces preload at any dose but only reduces afterload when higher doses are used. 📌 "High-dose" nitroglycerin is a bolus intravenous dose of 2,000 mcg every 3 to 5 minutes on top of a titrated infusion up to 400 mcg/min. 📌 The only study of high-dose nitroglycerin for acute heart failure syndrome showed that this strategy resulted in a significant reduction in: 1) the need for intubation, 2) the need for BiPAP, 3) the need for ICU admission. #criticalcare #pharmacy #medicine #clinicalpharmacy #pearls

End stage renal disease (ESRD) patients are often diagnosed with multiple pathologies like diabetes and hypertension. These disease processes often require management from other specialists. The multispecialty landscape many patients face can be confusing and convoluted. Studies have shown increases in clinical errors are directly correlated with an increase in specialists involved in a patient's care. This care fragmentation endured by many ESRD complicates the already invasive nature of traditional incenter hemodialysis. Imagine being an ESRD patient admitted to a post acute facility without dialysis. You see doctors and nurses daily who talk to you about your various diagnoses, provide complex care, and administer medications. Then you're placed in , often unreliable, contracted transportation to an incenter dialysis facility where you are likely surrounded by people you don't know. Home hemodialysis (HHD), or in house dialysis programs, can provide patients with more independence and agency over their lifesaving renal care, their schedule, and their time. In facilities with in house dialysis programs, the patients benefit from an improved continuity of care that does not require transportation and is performed by people the patient knows and trusts.
